1. Bovenhandse werpbeweging




6 fasen:

 wind-upfase
o voorbereiding  flexie
o optrekken contralaterale been (flexie in heup, knie en wervelzuil)
o schouders: endorotatie + adductie (ellebogen in flexie)
 stridefase
o bal loslaten
o contralaterale arm: adductie en endorotatie  abductie en exorotatie
o opgetrokken been naar voor in richting van slagarm
o romp: extensie-rotatie + lateroflexie
 arm-cockingfase
o neerzetten van opgetrokken voet  rotatie romp en benen  versnelling bal
o arm: horizontale abductie en exorotatie
 acceleratiefase
o schouder:
 van exorotatie naar endorotatie  belangrijk voor acceleratie!
 van horizonale abductie naar horizontale adductie
 einde: bal loslaten
 deceleratiefase
o versnelde arm afremmen
o flexie achterste been
o flexierotatie WZ
o schouder: eerst kleine abductie, daarna adductie + endorotatie
 follow-through-fase
o rotatie-nulstand
o overkruisen lichaam met arm in adductie
 veralgemenen
o back swing / achterwaartse acceleratie
 wind-up + arm-cocking + acceleratie
o voorwaartse acceleratie
o swing-out = follow-through
 meeste letsels tijdens arm-cockingfase en arm-deceleratiefase

 lage activiteit in schouder (trage beweging)
 hoge activiteit van UT, SA, anterieure deltoideus

 RC = GH-compressors en -rotators  laagste activiteit

 deltoideus helpt SS voor aanhouden abductie
 UT en SA  positioneren en stabilisatie scapula
o arm-cockingfase
 kinetische energie OL en romp overbrengen naar BL
 arm achter romp  roteert snel naar anterieur
 hoge spieractiviteit noodzakelijk:
 deltoideus  abductie
 RC  GH-stabiliteit
 IS, teres minor, lat dorsi  anterieure translatiekracht tegenhouden  kapsel en
ligamenten ontlasten
 endorotatoren (subscap, pect maj, lat dorsi)  exorotatie controleren door
excentrische werking
o pectoralis major en subscapularis  duale functie:
 concentrisch: adductie
 excentrisch: controle exorotatie
 continue verkorting & verlenging  ± gelijke lengte  isometrisch
 subscapulaire spieren  correctie stabilisatie en rotatie scapula
o protracters  controle retractie door: excentrisch + isometrisch 
concentrisch op einde van arm-cocking
 serratus anterior  maximale activiteit tijdens deze fase
 caput longum biceps + triceps  matige spieractiviteit voor schouder- en
elleboogstabilisatie
 verschil tussen gezonde pitchers en pitchers met chronische anterieure
schouderinstabiliteit:
 activiteit biceps en SS ↑
 activiteit pectoralis major, subscap, serratus anterior ↓
 toegenomen stretch anterieure kapsel  mechanoreceptoren prikkelen:
o stimulatie biceps en SS
 GH-stabiliteit ↑
 stress ↓ op inferieure GH-ligament bij abductie-exorotatie
o inhibitie pectoralis major, subscap en serratus anterior
 activiteit ↓ bij exorotatie excentrisch  stress ↑ anterieur kapsel
 risico op scapulamalpositie  impingement
o arm-acceleratiefase
 hoge-zeer hoge activiteit GH- en scapulaire spieren
o deltoideus  matige activiteit: onderhouden abductie (90-100°)
o endorotatoren (subscap, pect maj, lat dorsi)  hoogste activiteit concentrisch
o teres minor, SS en IS  matige tot hoge activiteit: correcte positie humeruskop

,  intensiteitsverschil tussen amateurs en professionals EN tussen gezonde pitchers en
chronische anterieure schouderinstabiliteit
o chronische anterieure instabiliteit:
 activiteit ↑ biceps, SS, IS  ondersteunende GH-stabiliteit
 biceps meer dan SS en IS  verhoogd risico op SLAP-laesies
 activiteit ↓ latissimus dorsi, subscap en serratus anterior 
worpefficiëntie ↓  blessures
o arm-deceleratiefase
 afremming acceleratiefase  grote krachten op schouder
 doel = overtollige kinetische energie absorberen (die niet aan de bol wordt doorgegeven)
 spierwerk:
o excentrische contractie posterieure keten (teres minor/major, lat dorsi, IS,
posterieure deltoideus)  afremming horizontale adductie + endorotatie en
distractie anterieure luxatie
o scapulaire spieren  hoge activiteit
 elevatie, protractie en rotatie scapula
o hoge activiteit  kracht en uithouding posterieure keten belangrijk
 tekort  risico op blessures (bv. RC-ruptuur, labrumpathologie, …)
 intensiteit professionelen tov amateurs:
o activiteit ↓ pectoralis major, latissimus dorsi, subscap, serratus anterior (zoals
arm-cocking en arm-acceleratie)
 zelfde activiteit biceps en RC tijdens deceleratie bij pitchers met en zonder chronische
anterieure instabiliteit
 biceps = grootste activiteit  2 functies:
o excentrisch: elleboogextensie afremmen  indien vermoeidheid: impingement
olecranon  botsporen
o synergisch met RC  distractie anterieure subluxatie voorkomen

 hand-entry  endorotatie en abductie  romp roteert
