DNP 825 Topic 2 Discussions 1 GCU Nov 2021   DNP 825 Topic 2 Discussions 1 GCU Nov 2021 Topic 2 DQ 1 How could you better implement descriptive epidemiological methods in your current or future practice? Provide an example of an instance when you might use descriptive epidemiology to improve care. Explain. Support your response with relevant literature. Descriptive epidemiology is the first step in studying a disease because it evaluates data about person, place and time. Personal information includes age, gender, race, ethnicity, education, socio-economic status, and occupation. Place relates to location such as work, residence, school, or recreation facility. Time include trends, such as: secular or long-term, cyclic changes or periodic fluctuations, and epidemic disease outbreaks or short-term fluctuations. Researchers reports that descriptive epidemiology aims to describe the distributions of diseases and determinants. It provides a way of organizing and analyzing these data to describe the variations in disease frequency among populations by geographical areas and over time (i.e., person, place, and time) [ CITATION Lon18 l 1033 ]. An example of instance when I may use descriptive epidemiology as a DNP prepared nurse would be studying the cause of increase rate of Catheter-Associated Urinary Tract Infections in patients with indwelling urinary catheter on my unit. Minimize urinary catheter use and duration of use in all patients, particularly those at higher risk for CAUTI or mortality from catheterization such as women, the elderly, and patients with impaired immunity (Centers for Disease Control and Prevention, 2015). First step would be to collect data on the demographic (age, race, gender, education) of the patients, time or frequency of infection rate (monthly, yearly, etc.), and then location (hospital units) of occurrence rate, and if catheter is inserted by a trained staff using sterile procedure. If patients are having increased infection rate, then questions need to be addressed as to patients who are being infected, how long it takes before catheter is remove from patients after procedure for which it was inserted, and if staff are following protocol for removal if there’s one, and if there is not, then there needs to be a change in standard of care by implementing a protocol for removal of catheter and reassessing to see if the change had a positive impact on the infection rate of patients. Working as an Advance Practice RN, it is important to understand the etiology of the infections and assist in finding a solution, adhering to careful and thorough ethical practice and procedure. Knowledge of how illness and injury are distributed within a population can provide valuable information on disease etiology and can lay the foundation for the introduction of new prevention programs [ CITATION Ann20 l 1033 ]. Topic 2 DQ 2 Which epidemiological study design would you use to evaluate an intervention in your current or future practice? Describe the population the intervention would affect and what the intervention would entail. Explain why the design you chose is the best option for this intervention and include a discussion of the design’s strengths and weaknesses in your response. Support your answer with relevant literature. My (Direct Practice Improvement (DPI) project is evolving as I continue to find the right direction to follow. I am currently contemplating a project in providing educational awareness to a community group about depression. My intervention is to provide individuals with educational resources that will enable participants to recognized depression as an illness and encourage them to seek help. I intend to utilize the Chronic Disease Self-Management Program workshop to educate participants for 2.5 hours weekly for 4-6 weeks. The teaching process makes this program effective. Classes are highly participatory. Mutual support and success build participants’ confidence in their ability to manage their health and maintain active and fulfilling lives (Self-Management Resource Center, 2020). Most people are afraid of stigma associated with depression preventing them from seeking help. Misunderstanding and lack of knowledge can thus result in public stigma, which in turn affects perceived/self-stigma which affects the ability to seek help. Yokoya et al., (2018) present a study of attitudes and public awareness of depression. According to previously done research, 46% of the participants sought professional help after obtaining education on the same. Therefore, to improve mental health, eliminating misunderstanding and increasing awareness is critical. A descriptive cross-sectional study will probably be the study design for this project. Descriptive epidemiology is used to describe the distribution of disease and other health-related states and events in terms of personal characteristics, geographical distribution, and time (Curley, 2020). This study design is usually less expensive and provide a timely data about the group being study. Since this project will be gear on studying the participants behavior relating to depression and how it affects their view on seeking help, it will determine if educational awareness will change their behavior and view of the disease allow them to seek treatment early. It requires less time and cost effective, but the weakness is that it does not provide us with the true risk of developing a certain disease outcome. Data from descriptive studies are readily available and can be taken from multiple several sources, making it inexpensive and timely. Curley, A. L. (Ed.). (2020).
