Task 2
This report is about how working practices are used to meet individual needs. It looks
further into how agencies work together to meet individual care and support needs, their
roles and responsibilities of professionals that are part of the multidisciplinary teams as well
as maintaining confidentiality and managing information using the three case studies
Nusrat, Patrick and Maria.


P6: Explain why meeting the needs of the individuals require the involvement of different
agencies.
Different agencies and professionals would need to work together to meet individuals
needs. The CCGs commission for most of the hospital and community NHS services in the
local areas that they are responsible for. This includes most community health services and
learning disabilities services. They also give patients and communities information on how
local services are developed. (NHS, 2019) This would help to provide high quality care to the
service users and meet the needs of individuals. In Nusrats case, one physical need of having
epilepsy and intellectual need of having learning disabilities. An EHC plan might be used to
provide more support for her special educational needs. Two different professionals such as
a GP and a nurse might work together to discuss and maintain her needs and support her. In
Marias case, a counsellor would explore alternative methods of falling asleep without the
use of medication since she suffers from a lack of sleep. Marias mother who has dementia
would benefit from being relocated into a nursing home as she would be provided with
specialist dementia care. It would also help Maria with her lack of sleep as it would reduce
the stress of caring for her mother whilst fulfilling her duties as a teacher. Since Marias
mother doesn’t speak English, an interpreter would be important as this means she is able
to be included in all areas of the arrangements being made without her feeling excluded
from the discussions. For Patricks case, the different professionals involved might include a
GP and a sexual health advisor as they work together by provide the best support in order to
meet the needs of Patrick and his partner.


P7: Explain the roles and responsibilities of different members of the multidisciplinary team
in meeting the needs of specific individuals.
A multi-disciplinary team is made up of professionals from the same service who have
different roles. These professionals work together to support individuals who are facing
complex situations. The multidisciplinary team supporting Nusrat would involve a GP and a
specialist nurse practitioner. The role of a GP is to treat all common medical conditions and
refer patients to hospitals and other medical services for urgent and specialist treatment.
The responsibilities include helping patients access specialist secondary care services when
needed and to ease difficulties of individuals with chronic conditions. The role of a specialist
nurse practitioner is to make a diagnosis and provide treatment, their responsibility is to

, record patient’s medical histories and symptoms and to create patient care plans. (Baileff,
2015) The GP has discussed ways in which Nusrats epilepsy could be managed and has
made a referral to the nurse practitioner who specialises in epilepsy that can help to
improve her uncontrolled epilepsy.
In Patricks case, the different members of the multidisciplinary team that meet the needs of
the individuals include the GP , sexual health advisor, counsellor and a nutritionist. The role
of the GP in this case is to conduct a series of blood tests before prescribing treatment. The
GP has also produced a treatment plan. The sexual health advisor helped Patrick and his
partner to understand and manage their condition. For example Simon has advised the
couple to use protection. The role of the counsellor is to encourage and help Patrick and his
partner to make their own decision. By active listening, the individuals would feel open and
comfortable with the counsellor which would result in an effective care plan and
subsequently benefit them. The counsellor can provide advice and encouragement to help
them become more confident without feeling judged and discriminated against because of
having HIV. The nutritionist will help to deliver, assess and generate professional advice to
improve Patrick and James’s health and wellbeing therefore promoting a healthy lifestyle
and diet. The nutritionist is beneficial to Patrick’s multidisciplinary team because Patrick’s
body would undergo changes due to the HIV and the medicine he is taking. The nutritionist
can advise and help Patrick with his diet by making improvements and deciding what is most
suitable for him. This will improve his quality of life as he will get important nutrients that
his diet could be deprived from.
The members involved in the multidisciplinary team for Maria include a GP, social worker
and a community care assistant. The responsibility of the GP is to review the patient’s
records and explore different ways to cure her insomnia without the use of medication. The
GP is also responsible for making a referral to social services to make arrangements that
would provide her mother with the care and support that she needs. The responsibility of
the social worker is to support Maria and her mother through difficult times and making
sure that vulnerable people like her mother are safeguarded and help to make
improvements in their lives. The role of the community care assistant is to assist Marias
mother with her personal needs such as washing, dressing and maintaining her hygiene, as
well as helping her with day to day tasks.
P8: Explain the arrangements for managing information between professionals.
Professionals manage information to other professionals by making sure that the
information being shared is only to the people who are entitled to have access. For example
people who are part of the multidisciplinary team. In the case of Nusrat, the GP, the named
person and the specialist nurse practitioner would have access to information about Nusrats
medical history as they would need to work together to provide the best care and support
that meet her needs. It is also important to share as much information as possible with the
service user and their family as this ensures that everyone is informed and reassures that
the best possible care is being given. Another way professionals manage information is by
keeping certain information locked away or password protected to prevent the service
user’s personal data from being accessed by others. The data protection act 1998 is an
