Lecture 4
Periodontitis is determined by the following AT patient presentation.
Severity
Complexity
Extent and distribution
It ranges from 1-4.
Severity is based on interdental CAL at the worst site
Stage I – 1-2mm
Stages II – 3-mm
Stages III or IV – 5mm or more
If the patient had RBL or tooth loss due to perio then that stage automatically gets bumped up
to stage 3 OR 4.
Complexity is based on local factors such as probing.
Special considerations –
Vertical bone loss at 3mm or more – Automatically stage 3 or 4
FI class II/III – Automatically stage 3 or 4
Less than 20 teeth remaining – Automatically stage 4

Localised – less than 30% of teeth involved in the severity/complexity
Generalised – 30% or more of teeth involved in severity/complexity
Molar/incisor pattern – classic localised aggressive perio
