1. Description of the patient: Age? Sex? Ethnicity? Marital Status? What precipitated hospitalization?
Number of days in the hospital? Mental Status, etc.
D.B. is 47-year-old Caucasian male. As per the patent, he was never married and his mother in
Colorado, and stays in the facility. He said he was admitted for 9 days already when we interview him. D.B.
said that he was admitted because he was trying to kill himself by cutting his throat and his wrist. His
mental status seems to be unstable as what we notce he is easy to snap and get mad and triggers him
thinking to slit his throat in front of the lobby of the facility.
2. Description of environmental setting where interaction took place. Explain the reasons for a supportive or
non-supportive environment. (e.g. noise, distractions, light, temperature, etc.)
The first tme we met DB was in the morning where his VS are being taken due to his complains
of chest pain. After that he was offered Motrin by the nurse which he refused. He then went out to smoke
where we had conversaton. Our conversaton was very casual. He seems calm after the thought that he
was having conversaton with other people (not a staff). He started telling a brief story about his situaton,
that he is currently homeless and lives on the street. He has a mother in Colorado, and he was never
married. Our first conversaton was very short, but he said he would see us again after lunch. The second
meetng happened at 1230, where we met him by the entrance of the smoking pato. This tme he was so
upset, that he mentoned that he met with his social worker, and he was very upset about their
conversaton. There were about four patents waitng for the smoke break. DB was very anxious in the
hallway and displayed distressed actons. DB spoke to us making it clear that he was upset at the staff. The
other clients listening very clearly to our conversaton. The moment they allowed us outside the,
environment was significantly supportve. It was warm outside, and we were able to talk to DB without
causing a scene, and he even made jokes to one of our members.
