Prediabetes continues to be a serious health issue in today’s society and education is key

to helping people avoid type II diabetes or prolong the time before it is diagnosed.(Burke, Sherr,

& Lipman, 2014)The object of this project is to increase prediabetic awareness and prevent or

prolong diagnosis by educating the public.

While developing a quality improvement plan in regard to prediabetes, the FADE model of

performance improvement was used to provide an in-depth explanation of a prediabetic health

program and a detail description of the resources that will be needed in order for this program to

be successful.



Evidence-Based Practice Plan Explanation



There is a very strong need for increasing awareness of prediabetes and more education

needs done for the public. There are more than 79 million adults in the United States with

prediabetes, and that number is expected to continue.(Sherr & Lipman, 2013) This is in effort to

prevent or prolong the diagnosis of type II diabetes in people today. In this improvement process,

ten potential candidates would be given a survey in regard to their potential prediabetic health

and of those ten, five people would be offered education thru a diabetic program at public health.

With this population, it is best to keep the group smaller, so mentors and staff are able to

concentrate on small number of people. These five people would be mostly underprivileged,

underinsured citizens of Webster County and have at least one or more social determinates that

would put them at risk to develop type II diabetes. The criteria in addition to answering the

survey would be : age 18-29, have the presence of prediabetes (either fasting glucose of 100-
