6635: Psychopathology and Diagnostic Reasoning Dr.
Subjective: CC (chief complaint): Since watching the recent school shooting at Ridgeview,
where 12 students and 3 teachers were killed, my wife has changed completely. Over the past
few months, she has become overly protective of our children, has difficulty sleeping, and even
when she does sleep, she wakes up within a few hours and loses her appetite. She seemed to have
"flipped a switch" in her thinking after this experience.
, Holliman was born and raised in Northern Ireland and is a 42-year-old Caucasian woman of Irish
descent. When she was 15 years old, she and her sisters moved to the United States to attend
college. Since the recent school shooting at Ridgeview that killed 12 students and three teachers,
she has undergone a dramatic shift in her personal and professional life. Due to what happened,
she has trouble sleeping and wakes up frequently during the night; her appetite has decreased
significantly; and she is glued to the television to keep abreast of the latest events. In the
interview, she stated that the school was no longer a safe place for children and that she would
never send her children to school or turn off the television again. She is adamant about
continuing her education. Following the incident, this patient's behavior has strained her
marriage with her husband, Greg. "We don't see eye-to-to-eye," she says. She's right. He claims
she has "flipped a switch" after this incident.
History of Psychiatric Treatment in the Past:
No history of mental illness, drug or substance abuse has been reported by the patient.
Medical or mental health hospitalizations are not acknowledged by the patient.
Only occasional use of Motrin and Tylenol was reported by the patient during the medication
trials.
Psychiatric Diagnosis: Patient denies any previous psychiatric diagnosis or history of
psychotherapy.
The patient denies any past or current use of alcohol or other substances.
As far as the patient is concerned, there is no family history of mental illness or substance abuse
in his family.
At the age of 15, Mrs. Holliman moved to the United States with her sisters to attend college in
