Situatie
Deze situatie speelt zich af in het ziekenhuis op de afdeling interne geneeskunde. Het doel is hier om
patiënten zo snel en effectief mogelijk te helpen bij hun aandoening, ziekte of klacht. Zodat ze weer
naar huis of naar een andere zorginstelling kunnen. De ziektebeelden op deze afdeling zijn erg divers.

De zorgvrager in deze situatie, dhr. X, is een man van 63 jaar. Hij heeft COPD gold 2 en Diabetes
mellitus type 2. Voor de rest heeft hij niks in zijn voorgeschiedenis. Dhr. is opgenomen vanwege een
