Major Depressive Disorder
● Five (or more) of the following in 2-week period to be diagnosed:
○ Weight loss and appetite changes
○ Sleep disturbances
○ Constipation
○ Fatigue
○ Worthlessness or guilt
○ Loss of ability to concentrate
○ Recurrent thoughts of death
○ PLUS—at least one symptom is also either
■ Depressed mood or
■ Loss of interest or pleasure (anhedonia)
○ If It last more than 2 years, it becomes chronic depression
○ May last a minimum of 2 weeks 6 months
Persistent Depressive Disorder
● Low-level depressive feelings through most of each day, for the majority of
days
○ At least 2 years in adults
○ At least 1 year in children and adolescents
● Must have two or more of the following:
○ Decreased appetite or overeating, insomnia or hypersomnia, low energy,
poor self-esteem, difficulty thinking, and hopelessness
, ● Substance-induced depressive disorder
○ Person does not experience depressive symptoms in the absence of drug
or alcohol use or withdrawal
● Depressive disorder associated with another medical condition
○ Can be caused by kidney failure, Parkinson’s disease, and Alzheimer’s
disease
○ Symptoms that result from medical diagnoses or certain medications are
not considered major depressive disorder
● Always want to assess for suicide thought..Do you have a plan?execution?
Implementation
● Three phases
○ Acute phase (6 to 12 weeks)
○ Continuation phase (4 to 9 months)
○ Maintenance phase (1 year or more)
○ Team work is the most important thing

● Selective serotonin reuptake inhibitors(SSRI)
○ Rare risk of serotonin syndrome
○ Citalopram - (Celexa)
○ Fluoxetine - (Prozac)
■ FDA approved tx in children and adolescents
○ Sertraline - (Zoloft)
○ Paroxetine (Paxil)
○ Pt teaching
■ ADR
■ Causes Nausea, HA, CNS Stimulation(Agitation, insomnia, anxiety)
■ Serotonin syndrome
● Cause by confusion, aitation, tachycardia, nausea,
abdominal pain, muscle rigidity, fever, tremor.
● Concurrent use with St john wort
● >= 2 SSRI
NOTE: WILL GET TESTED ON CITALOPRAM, FLUOXETINE AND SERTRALINE,
AMITRIPTYLINE
