M5 - Assess the benefits of multidisciplinary and multi-agency working for
specific individuals with care and support needs.

Multi-agency working is an effective way of supporting children and families with
additional needs and helping to secure real improvements in their life outcomes.
Multidisciplinary working is the mechanism for organising and coordinating health
and care services to meet the needs of individuals with complex care needs. The
teams bring together the expertise and skills of different professionals to assess, plan
and manage care jointly. The benefit of working in partnership is that all the
professionals are working together, communicating and planning as a team and
support is coordinated. Professionals can use their skills more effectively by
concentrating on meeting just some of an individual’s needs, rather than all of them,
and focus on what they do best.

Aisha
In Aisha’s case as she has dementia the following professionals and agencies will be
involved to work as a team in ordered to meet her needs, from the multidisciplinary
team the NHS professionals are, the geriatrician and the mental health specialist,
then from the multi agency team we have the local authority, the CCGs (clinical
commission group) and the Alzheimer's Society as we see this team includes different
professionals and services that all have different roles but the same duty which is
working together to meet Aisha needs. As we already know, the CCGs are responsible
for making sure that all the hospital, community and NHS services in the local area,
by choosing which services are needed for the diverse local population and ensuring
that they are provided. In this case the CCGs will be working with the local authority
which is an organisation responsible for the services and facilities that particular area
has to offer, in this case the multi-agency team will mainly be observing the services
and the environment of the hospital where Aisha is currently administered by
ensuring that the setting has enough founding and resources that are necessary to
support Aisha throwout his daily care, this will be only offered with the participation
of the multidisciplinary team of professionals that are currently assisting Aisha, which
most of them like the mental health specialist and the geriatrician are offered by the
local authority for individuals with physical, sensory or cognitive problems. Therefore,
together they will be able to decide what is more suitable for Aisha and what is not
by coming out with a plan that will be beneficial, furthermore the Alzheimer’s Society
will be working alongside the professionals and the local authority and the clinical
commission by offering guidance and advice that will help them all improve rights
and services for Aisha and other dementia individuals. However, working as a team
might bring some strengths and weaknesses. The strength of the multidisciplinary
disciplinary teams is that they will be sharing their own assessment with each other
in ordered to come up together with the right approach that will help to monitor

Aisha’s dementia and finding effective communication for the professionals to
communicate with Aisha, although the weakness of them working together could be
lack of cooperation from them which will immediately affect Aisha as she will feel left
out and not supported. On the other hand the multi agency team strengths is that
they will be able to offer the best service for Aisha by being able to solve her
problems easily by having the ability to take a holistic approach. However, the
weakness could be not effectively involving themselves to help meet Aisha needs by
leading other members of the team to be less capable to support her. The strength of
the multidisciplinary and multi agency working together are receiving coordinated
support from different professionals and services which work for the best interest of
the community and population by also being able to focus in one individual in this
case Aisha rather than more as this will bring the best outcome and will also allow
professionals and agency to be more confident and leeds pressure as they have the
right support needed. Although most of the time they might face some conflicts such
as poor communication as they all might have different opinions and ideas.

Valerie
In a health and social care environment you tend to come across multi-disciplinary
and multi-agency professionals working together to meet the service user needs by
working for their best interest. In the case of Valerie, her mental health problem and
obesity are the main issue that need to be overcomed, in order to do that the
multidisciplinary team which is the GP and the social worker , that has been
mentioned previously in this assessment they have been involved to work as a team
by sharing their skills and qualities in order to meet Valerie's needs. On the other
hand, the multi-agency team include the Health and Wellbeing Boards (HWB) as an
agency and the Clinical Commissioning Groups (CCGs) has the organisation that
worked alongside the health and well-being boards in order to provide her with
better quality health. The Health and Wellbeing Boards are statutory bodies that
promote and improve integration between practitioner working in the NHS which are
local healthcare, social care and public health and other related public service, this
then will be joint with the Clinical Commission Groups as they have a statutory duty
together as a multi-agency team to produce strategic needs, assessments, health and
wellbeing strategy in order to meet Valerie needs. This multi-agency team will then
work together with a multidisciplinary team of professionals who are currently
assisting Valerie by being able to work as a team to provide the best service. In order
to do that the Health and Wellbeing Boards and the Clinical Commissioning Group
will be supporting the multidisciplinary team in this case the GP and the social
worker, by ensuring that their are adapting Valerie's needs by commissioning a good
healthcare practice while being integrative to one and other, and by also assessing
the care setting by making sure that the care that has been offered to Valerie is
meeting her needs and if the outcomes are beneficial as well, this will allow the two
teams to have a better approach and an immediate response to the service user's
health and wellbeing. However, with every partnership working together there are
