Test 2 Module 3 Pre-hospital care for burns ● Remove victim from source of burn ● Stop burning process ● If chemical remove clothing and flush with large amounts of water ● If electrical and victim is in contact with source do not touch! Remove source with dry nonconductive object ● Establish patent airway (how do you do this out of the hospital?) and assess for inhalation injury-give O2 if available ● Start 2 large bore IV or IO with NS and LR ● Check peripheral pulse to assess circulatory status ● Assess and initiate treatment for injuries requiring immediate attention ● Remove tight fitting jewelry and clothing ● Cover burn with dry sterile or clean cover ● Cover victim with warm, dry cover to prevent heat loss ● Transport victim to nearest acute care facility ● Control pain Priority pt care for burns- skin, respiratory, GI, pain mgmt, hand hygiene ● Skin ○ Rule of nine ○ Minor burns ■ Soap and water ○ Moderate and major ■ Clean wound thoroughly ■ Assist with debriding necrotic tissue ● Mechanical- scissors and forceps ● Hydrotherapy- warm tub or running water, mild soap may be used ● Enzymatic- topical enzyme to break down and remove cells ■ Use aseptic technique ■ Guard against hypothermia during tx ■ Possible skin graft ● Respiratory ○ Ensure patent airway ○ Upper airway edema can begin 8 to 12 hours after fluid resuscitation ○ Supplement with O2 ○ May need mechanical ventilation ○ Cough and deep breath, incentive spirometer ○ Trach may be needed
