Task 2-working together in partnerships to meet needs
Meeting the needs of service users requires multiple agencies to be involved within their
healthcare in order to ensure that their dignity is maintained and that they feel listened to
which therefore ensures that they feel valued. This is called multi-agency work whereby
multiple agencies work in collaboration to provide support for individuals with different
difficulties and societal issues that impacts them holistically, but most significantly impacts
their health and wellbeing. Issues such as an individual living in a deprived area can cause
adverse effects such as respiratory difficulties. In addition to this, financial matters can have
a profound impact on individuals, for example, living in an area with high unemployment
can greatly impact an individual's health as they are financially unable to afford public
transport to get to health care settings and purchase healthy foods which in turn affects
their diet increasing their weight and in turn their susceptibility to developing chronic
diseases. To minimize these conceivable issues and risks agencies work in collaboration to
ensure that individuals needs, and choices are effectively met in health care settings. This is
effective as if agencies didn’t work together healthcare wouldn’t be improved and care
provision wouldn’t be effective. The introduction of diverse institutions and agencies ensure
that health inequalities are reduced protecting the health of vulnerable individuals.
Institutions such as the Clinical Commissioning group in England, commission the majority of
healthcare settings from the NHS services in England that may include hospitals, pharmacies
and dentists. This institution was replaced by the primary care trust, following the
introduction of the Health and Social care Act 2012, which aims to improve the
responsibilities in health and social care settings whereby health professionals hold the skills
required to successfully plan health care services in England which assures adequate service
is provided. In order to improve care provision, CCGS have the responsibility to assess the
requirements of community NHS services so that new regulations can be provided and
implemented into health care settings by ensuring that the planning of mental healthcare,
community care, rehabilitative care, emergency care and community care are fulfilled which
in turn maximizes the quality of care provided and better provision for service users. The
introduction of the CCGS is effective as they gather evidence and information on how to
meet the needs of individuals that ensures a personalized approach so that better health
and wellbeing practice is promoted through listening and supporting service users needs,
wishes and wants. This therefore means that health professionals improve as they acquire
the necessary skills and knowledge that work as an asset to the development of high-quality
care provision.
Moreover, there are bodies that are responsible for working in partnership with health and
social care by being involved in service provision. An example of this is HWB (health and
wellbeing boards) which was introduced by the Health and Social Care Act 2012, this body
aims to determine all the requirements from diverse health and social care services through
research and collection of information, so that trends, patterns and correlations of ill health
are identified and can therefore be utilized to find strategies and studies on how to meet
the populations needs in the best way possible way. Therefore, ill health and inequalities of
health are further minimized as consisting of skills such as commissioning, leadership and

, integration encourage bodies, institutions and agencies to work together effectively and
provide appropriate care and service provision. With support provided by different directors
of specific services, the HWB will provide an assessment called the JSNA (the joint strategic
needs assessment) which consists of demographic data on a wide range of issues and
inequalities whereby needs can be established and the JHWS (joint health and wellbeing
strategy) is produced which states the needs that have been met. The CCgs then further
observe this and then a final plan is formed on the steps and stages that should be followed
by publishing it so healthcare settings acknowledge what is required of them in order to
follow the plan effectively.
Moreover, role assessments exist as they enable health care professionals to identify service
users' issues and fill a care plan that reflects their choices in order to overcome those issues,
assuring adequate care is provided. This is done through policy frameworks which asses the
individuals need but also identifies whether they are eligible to access help and support
through health services. This looks at the social determinants such greatly impacts an
individual's life such as living alone or having a low income which are major factors to look
at whilst looking at the eligibility and health assessment. Policy frameworks aim to
effectively integrate with health and social care settings and further minimize inequalities by
focusing on the prevention of ill health. An example of a policy framework consists of the
CAF (common assessment framework) which is a process that allows healthcare
professionals to identify individuals care needs by doing an assessment which state the
different choices preferences and issues to do with their health assuring an holistic
approach to their care.
Some service users who require support may have multiple and complex needs Therefore,
working in collaboration is essential as it ensures that the vulnerable individuals healthcare
needs are effectively met, and adequate services is provided. A multi-disciplinary team is
made up of health care professionals who specify in different disciplines and work in
collaboration to better the quality of life for the patient.Multi- disciplinary teams are
effective as it consists of members who work in partnership to provide adequate services.
For example, doctors, midwives, and pediatric nurses may work together, collaborating
within the maternity department whereby they are all aware of each other's roles and
responsibilities which enables them to work within their competency promoting a holistic
approach to care. Effective multi-disciplinary teams require effective inter-disciplinary
communication which consists of active listening, teamwork and leadership as well as
holding interpersonal attributes such as a positive behavior and attitude. It is exceedingly
major that these skills and attributes are implemented into multi-disciplinary teams as it
minimizes conceivable conflicts between health professionals or perhaps confusion due to
misunderstanding as a result of ineffective communication negatively impacting the overall
care provided.
