i. Primary: drivers license, passport, identity card, Indian status card, permanent resident card, BC
services card
ii. Secondary: care card, birth cert, Canadian citizenship, permit, naturalization/marriage/name
cert, consular ID card
- Identify PTs using at least 2 person specific identifiers: full name, home address, DOB, PIN, photo
Hospital Pharmacy Technician
- 2y experience or tech course prior to Jan 2011
- Annual audit
- Retain record of training x 3y
- Sterile products min of 2 re-audits
- May NOT: check transcription accuracy, approve stability/compatibility information, calculations
Narcotic Counts
- Counts/reconciliations done: min q3mo, change of pharmacy manager, break-in/robbery, drug
diversion, closes/ceases business, security compromise
Record: name, strength, quantity, DIN
- Count not to be conducted by same person who purchases
- Keep records for 3y
- Report losses within 10d to Health Canada, 24h to eServices
Community Manager Education
- No later then one year after appointment and every 3y
- Ensure staff retrained on security annually
- Must have written policy procedure to verify identity/registration of applicants
