Three Components of Value-Based Care

Value-based care is a healthcare model that links financial rewards to patient health

outcomes rather than to the number of services provided. In this model, healthcare providers are

compensated for improving patient health, successfully managing chronic diseases, and

preventing complications. The model is supported by several key frameworks, including the

Institute for Healthcare Improvement’s (IHI) Triple Aim, the six domains of healthcare quality,

simultaneous pursuit of three components: improving the experience of care, improving

population health, and reducing per capita healthcare costs (Berwick et al., 2008).

The effectiveness of one of the primary frameworks supporting value-based care, the

IHI’s Triple Aim model, is illustrated by a 12-month managed care program implemented by a

Midwestern institution’s community healthcare division for underserved individuals diagnosed

with heart failure and/or diabetes. The program demonstrated significant improvements in

population health, as participants achieved better control of chronic disease indicators and

became more engaged in managing their own care. In addition, the program enhanced the patient

care experience through education and social support initiatives that fostered trust, reduced

missed appointments, and helped mitigate challenges associated with rural healthcare workforce

shortages. Finally, the intervention contributed to lower per capita healthcare costs by generating

a positive return on investment through reductions in emergency department visits and inpatient

utilization (Fawcett Jr. et al., 2018).

improve the structure of healthcare systems, enhance healthcare delivery processes and outputs,
