The Counselor's Ethical Role With Clients Using or Considering Psychotropic Medications

Clients rarely arrive at counseling untouched by the question of medication. Many are

already taking psychotropic medications, and others begin considering them partway through

treatment. Depression, anxiety, bipolar disorder, attention-deficit/hyperactivity disorder,

psychosis, trauma-related disorders, and substance use disorders are all commonly treated with

medication, counseling, or the two combined (Sinacola et al., 2020), which means counselors

need a working knowledge of psychopharmacology. Being informed, though, is not the same as

being qualified to prescribe or manage medication. The counselor's responsibility is to stay

within professional competence while supporting client autonomy, monitoring symptoms,

addressing stigma, making appropriate referrals, and collaborating with medical providers

(American Counseling Association [ACA], 2014; Sinacola et al., 2020).

Counselors and Prescribing Authority

The short answer to whether counselors should prescribe is no, not without separate

medical training and legal prescribing authority (ACA, 2014; Sinacola et al., 2020). Counseling

programs teach assessment, diagnosis where state law permits, counseling intervention, and

collaboration with other providers. What they do not teach is medicine: conducting physical

examinations, interpreting laboratory findings, weighing medication interactions, selecting

medications, or setting dosages. Those skills require specialized education in pharmacology,

physiology, contraindications, side effects, and medical monitoring that standard counselor

preparation does not include (Sinacola et al., 2020).

The ACA Code of Ethics draws the same line. Standard C.2.a. limits counselors to

practicing within their education, training, supervised experience, credentials, and professional

competence, and Standard A.4.a. directs counselors to avoid actions that risk harming clients
