Protected Health Information
Protected Health Information (PHI) is information which can be used to identify a patient
and that relates to his/her health status, treatment, or payment for service (Amin et al., 2024). In
the ICU, PHI would include patient names, medical record numbers, diagnoses, ventilator
settings, vital signs, medications, and even indirect identifiers like room numbers or visit times.
The Health Insurance Portability and Accountability Act (HIPAA) guarantees the security of PHI
by requiring that health care providers maintain privacy, security, and confidentiality. To the
ICU staff, this means sharing patient information with only authorized team members who will
be involved in care, avoiding making inattentive remarks about patients in public areas, and
never putting patient information on the internet. Because ICU patients are often very sick, even
minor disclosures can unintentionally reveal them and destroy families. Adhering to HIPAA
protects trust, protects vulnerable patients, and upholds ethical, professional practice in this high-
risk environment (Amin et al., 2024).
Privacy, Security and Confidentiality
Confidentiality, privacy, and security are the pillars for the protection of patient
information in health care. Privacy refers to the patient's right to control whether and how his or
her health data is used or disclosed. Confidentiality is the obligation of health professionals to
keep patient data confidential and disclose it only to those entitled to know. Security is the
technical and administrative methods put in place to protect health data from unauthorized use,
access, or destruction (Hulkower et al., 2020).
Use of technology in the ICU, where patients are seriously ill, creates unique risks.
Electronic health records (EHRs), for example, could remain open on desktops, exposing
confidential information. Confidentiality could be breached if patient progress is openly shared
in corridors or elevators among staff. Confidentiality can be breached if a nurse tweets about "a
young trauma case in bed 4" even without mention of a name. Security threats include insecure
passwords, spoofed emails or not logging off patient data accessed by medical equipment.
Interprofessional teamwork is required because nurses, physicians, respiratory therapists,
and pharmacists all handle PHI. Patient data is safeguarded by constant communication,
coordination, and shared responsibility (Ibrahim et al., 2024). When coordinated, groups of
