The ANA defines assessment as a systematic, dynamic way to collect and analyze data about a client, the first step in
delivering nursing care.
Assessment includes not only physiological data, but also psychological, sociocultural, spiritual,
economic, and life-style factors as well.
A complete assessment involves a review and physical examination of all body systems. Which also includes cognitive,
psychosocial, emotional, cultural, and spiritual components and is appropriate for a
patient with a stable condition who is not in a acute distress.
A focused assessment is advisable when the patient is critically ill, disoriented, or unable to respond. It is used to gather information about
a specific health problem. For ex: pt reports abdominal distention, lack of appetite,
and straining to have a bowel movement, one possible problem statement might be
"Infrequent or difficult bowel elimination."
Also outcomes is part of this assessment.
