VSIM Li Na Chen / Mental Health Case: Li Na Chen, Part 2 - Document findings related to the screening of Mrs. Chen with the Hamilton Rating Scale for Depression. (All Answered) - Mrs. Chen spontaneously reports feelings of depressed mood, sadness, hoplessness, or worthlessness. She experiences feelings of self-reproach and feels that she has let people down. She feels that life is not worth living. Mrs. Chen complains of occasional difficulty falling asleep and complains of being restless. While there are no episodes of insomnia, she did stop working because of her present illness. There is obvious retardation in her interview, but she is fidgety. She worries about minor matters. There is an absence of somatic anxiety and, while she does have a loss of appetite, she is eating without staff encouragement. She reports heaviness in her abdomen, limbs, back, and head as well as backaches, headache, muscle aches, and a loss of energy and fatigability. There are no genital symproms or hypochondriasis. She reports a definite weight loss, but does not feel depressed. There is no depersonalization, derealization, paranoid or obsessional and compulsive symptoms. She elicits feelings of helplessness only by inquiry. She consistently feels “hopeless” but accepts reassurance and indicates feelings of worthlessness only on questioning.
