1. Review the detox process. What would be the first priority?
a. Symptoms that occur upon reduction or discontinuation of a substance that has
been used regularly over a prolonged period.
b. Symptoms are specific to the substance used.
c. Disruption in physical and psychological functioning with disturbances in
thinking, feeling, and behavior (may have suicidal thoughts because of
withdrawal).
d. Priority is SAFETY.
e. Alcohol withdrawal:
i. 4-12 hours of cessation.
ii. Symptoms: tremors, insomnia, elevated BP, irritability, sweating.
f. Amphetamine/cocaine withdrawal may result in dysphoria, fatigue, sleep
disturbances, and increased appetite.
g. Withdrawal from caffeine may include headache, fatigue, drowsiness, irritability,
muscle pain and stiffness, and nausea/vomiting.
h. Withdrawal from nicotine may include dysphoria, anxiety, difficulty
concentrating, irritability, restlessness, and increased appetite.

i. Withdrawal from short-acting drugs (heroin):
i. Symptoms occur within 6-8 hrs.
ii. Peak within 1-3 days.
iii. Gradually subside in 5-10 days.
j. Withdrawal from long-acting drugs (methadone):
i. Symptoms occur within 1-3 days.
ii. Peak between days 4 and 6.
iii. Subside in 14-21 days.
k. Symptoms of opioid withdrawal:
i. Dysphoria, muscle aches, nausea/vomiting, lacrimation or rhinorrhea,
pupillary dilation, piloerection, sweating, abdominal cramping, diarrhea,
yawning, fever, and insomnia.
l. Cannabis withdrawal:
i. Symptoms occur within a week following cessation of use.
ii. Symptoms include irritability, anger, aggression, anxiety, sleep disturbances,
decreased appetite, depressed mood, stomach pain, tremors, sweating, fever,
chills, or headache.
2. Review what was stated on taped lectures about alcoholics and pain control.
a. Patients who are addicted to alcohol/benzos may develop cross tolerance to
analgesics.
b. Will request more pain meds because of tolerance to analgesics.
3. What pt outcome, related to AA, would be the most appropriate for the nurse to discuss
with pt?
