Week 1: Models of Transcultural Nursing Larry Purnell’s theory on cultural competence came about when trying to teach students to create a framework that will help them understand the cultures of patients. Purnell’s theory on cultural competence is illustrated using a circular model that includes seven different sections or rims. The rims from outer to inner are as follows, global society, community, family, person, 12 domains, a saw tooth line that represents cultural consciousness, and finally, the empty center that represents what we do not know about culture. “The 12 domains comprising the organizing framework are briefly described along with the primary and secondary characteristics of culture, which determine variations in values, beliefs, and practices of an individual’s cultural heritage” (Purnell, 2002). Basically, Purnell suggests, cultural competent health care providers are aware of self, personal beliefs and morals, environment and existence. Having self-awareness will prevent inability to deliver cultural care. A person’s culture is certainly a huge influence in how a person seeks, perceives, and receives care. By incorporating Purnell’s theory when assessing patients, it will aid increasing cultural competent care. Healthcare providers who are consciously competent will “learn about the client’s culture, verifying generalizations about the client’s culture, and providing culturally specific interventions” (National Association of School Nurses, 2013). Assessment and intervention plans that are culturally competent will improve patient care that is going to be delivered. It guides nurses to format a plan of care that is unique and individualized. This will allow a goal and action that is understood and more likely to be achieved when shaped to a patient’s cultural belief and needs. Learning about culture is a continuous process that is best learned through encounters with diverse cultures.
