Service Line for Wound Care Services

Proposed Nursing Service Idea

The proposed nursing service idea would be a wound care service that will provide
specialized treatment and education for patients with acute and chronic wounds, including
diabetic ulcers, pressure injuries, and venous stasis ulcers. Services will be offered through
outpatient clinics, home health visits, and telehealth consultations to improve accessibility
and continuity of care. The program will also emphasize patient and caregiver education
on prevention and self-management strategies, while collaborating with interdisciplinary
providers such as dieticians, podiatrists, and rehabilitation specialists to promote healing and
reduce complications. By addressing the growing needs of high-risk populations, this service
will improve patient outcomes, decrease hospital readmissions, and lower long-term healthcare
costs.
Implementation of the wound care service will require strong leadership skills,
particularly transformational and servant leadership approaches. Transformational leadership
will help inspire staff with a shared vision for patient-centered care and support innovation in
wound management practices (Boamah et al., 2018). Servant leadership will ensure that the
needs of patients and staff are prioritized, fostering collaboration, compassion, and supportive
environment (Eva et al., 2019). A diverse staffing model that includes wound care-certified
nurses, advanced practice nurses, and assistants from varied cultural and professional
backgrounds will promote equitable and culturally sensitive care. Effective communication,
cultural humility, and strategic planning will be essential for leaders to create a sustainable
program that delivers high-quality, accessible wound care.

This wound care service will be important to the population of Harris County, where
chronic and complex wounds disproportionately affect vulnerable groups such as older adults,
individuals with diabetes, and those with limited mobility or poor access to specialized care.
Harris County is one of the most populous and divers counties in the United States, with
significant health disparities linked to socioeconomic status, race, and ethnicity (Houston
State of Health, n.d.). These disparities contribute to higher rates of chronic illnesses such as
diabetes, which increases the risk of non-healing wounds. Without proper management, wounds
can quickly progress to severe infections, prolonged hospitalizations, or even amputations,
significantly impacting patients’ quality of life. By offering accessible, evidence-based treatment,
along with education and preventative strategies, the service line will directly address these
challenges in Harris County. Providing culturally sensitive care through a diverse staffing model
will help overcome barriers to access and build trust within underserved communities. This
approach not only improves individual outcomes, but also lowers hospital readmission rates,
reduces overall healthcare costs for local health systems, and contributes to healthier
communities across Harris County.
