Service Line for Telemedicine Follow-up for Wound Care


Proposed Nursing Service Idea


My proposed nursing service line plan is to implement follow-up for wound care through
telemedicine for individuals who need wound care but have limited or no resources for transportation
to in-person wound care. In Wharton, Texas, the percentage of people over 65 is 18.6 compared to
13.7% for Texas. Wharton County rates 24% of its population in poor/fair health when compared to
17.1% in the United States (County Health Rankings & Roadmaps, 2025). The county is mostly rural with
limited clinics and hospitals. County Health Rankings & Roadmaps have Wharton County with 1 primary
care physician for 2,610 people, which is double that of the United States, with 1 primary care provider
for every 1,330 people. OakBend Hospital System has a small hospital in Wharton City, TX, that services
the area, but their wound care services are located at their 2 Richmond, TX, campuses. This nursing
service line would be a transitional care program that will be nurse-led, with a nurse leader who would
be designated to oversee the program and would include program development, implementation, and
evaluation. The nurse leader would lead an interdisciplinary team promoting a patient-centered
approach to wound care.

The service line would be led by two certified wound care nurses and a part-time billing coder who
would work within this team. This service line would be an extension of the wound care clinic that
OakBend has established at its Jackcon St campus. The initial visit would be in-person at the wound care
center, where a plan of care would be established by the provider. If the patient is determined that
telemedicine for follow-up wound care would benefit them due to a lack of reliable transportation, they
would become a patient within the service line. A meeting with the certified wound nurse who would
be monitoring care would take place at the in-person visit. The wound nurses who would monitor
follow-up care through telemedicine and document data into the patient’s chart. Documentation may
include a photo of the wound from the telemedicine visit, along with data needed. The billing coder
would be responsible for billing for telemedicine wound care services. That person would have extensive
knowledge of billing codes and use the knowledge to give feedback to the nurses to help ensure
reimbursement. This role is important, and the billing coder will be expected to attend in-services and
keep up-to-date on what documentation will be needed for reimbursement from Medicare and other
insurance (Song,2025). The three members of the service line will meet weekly. Criteria for the service
line will be leg wounds, over the age of 65 years, who do not have reliable transportation, and home
healthcare visits may not be available to them. The patient will have an in-person visit at the wound
center and a plan of care, with the telemedicine wound nurse will establish goals. A physician and/or
nurse practitioner at the wound clinic would set individual care plans for patients with a clear strategic
vision for a goal in wound care and assessment of the wound. This will be accomplished through
effective communication, critical thinking, and logical decision-making, placing the patient’s care and
experience at the frontline. The service line proposed will assist with follow-up wound care through
telemedicine, which will include a certified wound care nurse conducting an assessment with the
patient.
