-General: if patient refuses medication, review rationale for use
of drug, explain the risk for refusal, review the information that
may be appropriate, and if med will not be taken by the patient
document every attempt/information disclosed and contact care
provider.
-Eye drops are used for pupil dilation or constriction, treatment
of glaucoma (intraocular pressure). Put on gloves, offer patient
tissues to remove tears that may spill, clean eyelids and eye of
drainage with gauze squares moistened with normal saline as
needed. Tilt patient head back slightly if sitting or if lying
support head with pillow. Remove cap from medication bottle,
do not touch cap or inner side of the container, invert container,
have patient look up and focus on something on the ceiling,
place thumb or 2 fingers near margin of the lower eyelid
immediately below the eyelashes, apply pressure downward over
bony prominence of the cheek. The lower conjunctival sac
exposed as lower eyelid is pulled down. Hold dropper close to
eye but avoid touching eyelids or eyelash. Squeeze container to
, allow the number of drops prescribed to fall into the lower
conjunctival sac. DO NOT let medication fall into the cornea.
This may cause an unpleasant sensation. Release eyelid and have
patient close the eye gently. Apply gentle pressure with gloved
finger over inner canthus to prevent the eye drops from flowing
into the tear duct, this minimizes effects from the medication.
-Replace cover, instruct patient not to rub affected eye, remove
PPE assist patient to comfortable position. Document
administration date/time/dose/route/site on the eMAR/MAR

Allergic reaction NSG124.01.02.04
-Allergy reaction is an immune response, the intensity of an
allergic reaction can range from mild itching to severe rash to
anaphylaxis.
-Anaphylaxis is a life threatening response characterized by,
bronchospasm, laryngeal edema and a precipitous drop in blood
pressure. Less than 10% of adverse drug reactions are of allergic
type.
