Community Windshield Survey Form
Directions: Refer to the Community Windshield Survey Guidelines and grading rubric for specific instructions in order to complete the information below. 1.This assignment is 8 hours of MANDATORY scheduled clinical hours. 2.NO POINTS ARE AWARDED BUT PARTICIPATION IN WINDSHIELD SURVEY AND COMPLETION OF THIS ASSIGNMENT ARE MANDATORY AND CONSIDERED INTO THE OVERALL CLINICAL EXPERIENCE AND ARE “PASS/FAIL”.

1. Introduction of Community (P/F)
Identify the city and state of your community and briefly describe the community you will be using for this assignment. It should be the area where you live or the area
surrounding your work setting but must include a residential area. Demographic data are not
needed.The area our group chose for our windshield survey is in the area of North Las Vegas with the zip code 89115. This are is a little unique to Las Vegas, in that this is the area that includes Nellis Air Force Base. The Base is gated and guarded as well as the off base housing provided for military and family. This zip code is comprised of areas of North Las Vegas, Nellis Air Force Base, and Sunrise Manor, Nevada. This area is closer to the area where I attend school, and is an area I was not very familiar with prior to embarking on this group project.
Our group thought it would be interesting to explore the area that included the military base, the nice military housing immediately surrounding it, and the much poorer area just outside that. What I found
most interesting in our first drive thru (Gabby’s boyfriend is military and was able to get us on base to drive around) was the on base housing is not kept up very well.
