1) Ureum
Synoniem:

2 moleculen NH3 (ammoniak) + 1 molecule CO2
=> Gevormd via de ureumcyclus



NH3 komt voor als NH4+ (ammonium) bij pH 7.4


Ammonium:
- Afkomstig uit de afbraak van eiwitten:
- Exogeen: Eiwitten uit de voeding + Digestieve proteasen -> Vrije AZ -> Lever: AZ -> Ammonium -> Ureum
- Endogeen: Afbraak van weefseleiwitten -> Vrijzetting AZ -> Lever: AZ -> Ammonium -> Ureum
- Toxisch voor cellen
-> Zo snel mogelijk transporteren naar de lever
- Lever:
- Ammonium wordt gebruikt voor synthese van glutamine, glutamaat en carbamoylfosfaat
- Carbamoylfosfaat wordt omgezet tot ureum (ureumcyclus)
- Opstapeling?
- Veel eiwitten worden afgebroken OF Er is iets mis met de leverfunctie
=> Het is een maat voor de leverfunctie

Ureumcyclus:
- 2 moleculen ammonium + 1 molecule CO2
- Enkel door de hepatocyten (bezitten als enige arginase)

Ureum:
- Komt in BB
- Vrij diffundeerbaar door de meeste celmembranen
-> Wordt teruggevonden in vrijwel alle intra-en extracellulaire compartimenten
- Eliminatie:
- Nieren
- Bacteriële afbraak in de darm (door intestinale ureasen)
2x Ureum + urease -> Ammonium (scheet)
- Gefiltreerd in glomeruli
- 40 – 80% passieve terugresorptie in de proximale tubuli
-> Afhankelijk van de ureumconcentratie, urine-debiet en hydratatietoestand
- Ureumconcentratie: Hoe meer ureum aanwezig, hoe meer terug geresorbeerd moet worden
- Hydratatietoestand: Ureum wordt mee geresorbeerd met water. Bij gedehydrateerde toestand willen we
geen water verliezen -> Veel terugresorptie van water en dus ook van ureum -> Weinig urine

Ureumspiegel:
- Ruwe indicator voor renale functie (grootste hoeveelheid geëlimineerd door nieren)
- Ureum in plasma: Afhankelijk van adequate renale doorstroming en functie
- Ureum / creatinine ratio: Nierinsufficiëntie
- Niet-renale factoren: proteïne-katabolisme, voeding, hydratatietoestand lichaam en leverfunctie
-> Ureum is geen goede parameter voor de nierfunctie omdat vorming van ureum ook afhangt van de
leverfunctie en de concentratie hangt ook af van alle niet-renale factoren.
