Executive Summary
The proposal focuses on creating custom infectious disease forms for use in the SYSTOC
EMR. The rationale for this proposal is to provide custom infectious disease forms for
contagious diseases for our employees who are exposed to or those that contracted a contagious
disease while working. The proposal will improve patient (employee) satisfaction and provide
employees and staff with a better understanding of their infectious diseases.
Planning and Analysis
The planning and analysis phase completed during the project’s first phase illustrates the
organizational need and desire to incorporate clinical decision support into their current system.
The initial phase outlines the goals and overall project scope. Teams members and their roles are
broadly established since the need for the project has been established.
Designing and Building
The designing and building phase has most of the project work. Data is gathered and
reviewed. This phase requires group input to be successful. This phase supports, even more, the
need for this project as the forms are visualized
Implementing and Analysis
The implementation and analysis phase identifies task owners and their responsibilities
for tasks using a Gantt chart. It also outlines the project timeline for an overall view of all
stakeholders. The project is tested during this phase.
Monitoring, Maintaining, Supporting, and Evaluating
During the final phase of the system development lifecycle, the maintenance and
monitoring plan is outlined, executed, and delivered. Systems are set up for training EHS staff on

infectious disease forms. Finally, soliciting user feedback on the delivered project goals is
obtained.

Employee Health Infectious Disease Documentation in Systoc
UMass Memorial Health, Employee Health Services (EHS) supports over 16,000
employees in Massachusetts and those remote as far as Florida with their employee-related
incidents, exposures, etc. EHS uses SYSTOC EMR for employee documentation vs. EPIC.
SYSTOC platform uses PDF forms to capture and write data to the backend database. EHS does
not currently have any consistent way to capture infectious disease information from employees
when they call with an exposure or known contagious disease diagnosis.
This project aims to close this gap and allow for all disease information, transmission,
incubation, treatment options, etc., starting with scabies, to be consistently gathered and provided
to infectious disease specialists to assist in reducing the spread of contagious disease among our
healthcare providers and patients. The form will also include education about the disease, how
the disease is transmitted, incubation time, infectious period, critical exposure criteria,
prescription treatment plan, EHS follow-up needed, and return to work criteria. By having a PDF
form for each disease, starting with scabies, consistently gathering the purposeful information
will give a complete picture of the potential spread and other possible exposed individuals that
may require treatment. This information needs to be collected consistently so the spread of
disease is happening.
Establishing a system is necessary to gather employee exposure information or their
exposure to other patients or healthcare workers. This project aims to close that gap by having
PDF forms for each disease frequently seen in a healthcare environment, starting with scabies.
These PDF forms will contain employee and exposure information, i.e., patient, coworker, travel,
etc. It will also educate employees about the disease, incubation period, disease transmission,
