Unit 5 Task 2:

P6- Explain why meeting the needs of the individual requires the involvement of different
agencies.
D4- Evaluate how multi agency and multidisciplinary working can meet the care and support
needs of specific individuals.

The agencies involved when meeting needs of individuals are:
Clinical commissioning groups: this consists of doctors and other health care
professionals and their knowledge is needed when providing care to patients with specific
needs, the individual has a choice of how they would like to be cared for with resources and
preferences accounted for.
Local authorities: this where public health services work together to improve the public’s
health and encourage them to lead healthy lifestyles. They can meet the individuals needs
by promoting preventive services providing service users with adaptations to their home with
their specific needs.
Health and wellbeing boards: these have been set up as the leaders of health and social
care can assess the needs of the population and figure out strategies for local authorities.
Their role is to assess and think about current and future needs of the population. Members
such as the local authority director meet the needs of the individual by addressing the needs
and providing them the service they need.
Common assessment framework (CAF): this is the process to identify children who have
additional needs and strengths to provide them with a multi-agency support plan so that their
needs are met by different professionals. This is usually used for children and young people
up to the age of 18 and can be extended from that age where appropriate. CAF members
consist of GPs, police, SENCO and social workers, they help meet the needs of vulnerable
children by putting in place interventions that will help support them by their individual needs.
The national framework for NHS continuing healthcare: this means a package of
ongoing care is given by the NHS where the individual has been assessed and found to
have a health need, this ensures that individuals are provided with support and this can be
provided for them. And this continuing care can be given when individuals are residing in
hospital or a care home and outpatients too.

When working with service users with different needs, each person will require involvement
of different agencies, this is because there may be more than one issue that should be dealt
with. Different multi agencies can help an individual change there to help with problems
individuals have. In Nusrat's case, she would have her own personal care plan at the centre
that she goes to and this can help to deal with her issues and in this way she can progress.
As Nusrat has learning disabilities, epilepsy and communication difficulties, the centre can
help to deal with her issues and problems she may face. The plan includes a counselling
skills course which can help Nusrat with her communication difficulties as they will help her
learn new skills like makaton which is a sign language. She has also been referred to a
specialist practitioner by the G.P who is a professional that knows alot about epilepsy. This
shows that Nusrat goes to see another agency which is specifically just for epilepsy which is
good as they will concentrate on that to help her get better at a faster rate. The clinical
commissioning groups consist of doctors and other health care professionals which have
been mentioned above that help patients with their needs. Each individual or patient
receives the best care needed for them with the best resources.

, Alice Fernandez who is an alcoholic and is on antidepressants, she was referred to an
aerobics class which holds a meeting called “alcoholic anonymous”, this agency is to help
Alice express her feelings and speak about her insecurities and why she is addicted to
alcohol. And as there are other people there who are also addicts, their story will make her
feel more comfortable in expressing her feelings and it is suitable for Alice. Alice will also
need to change her lifestyle as her consuming alcohol affects the liver and this way she
visits the G.P and a nutritionist who will help her with her diet to make sure her health
improves. She was referred to a specialist at NS trust as they are concerned with her liver
function. The G.P referred her to the NHS trust and the G.P can do a blood test and provide
medicine however the NHS trust have more facilities in which they can help Alice. Another
agency which is involved is the local housing association as Alice can benefit from this by
receiving the support she needs considering her situation. The clinical commissioning
groups are involved as well as the national framework for NHS continuing health care. This
is ongoing care which is provided for the individual, this framework ensures the individual
receives the right package and that their needs are met, they provide patients with care with
hospitals and care homes.
Martin Smithers has been made redundant from his factory job, and this has caused him to
not pay household rent and fears he may be evicted from his apartment. Because of this his
skin condition has become worse. His class at the centre suggested he make an
appointment with the debt advisor. The advisor produces a plan to enable Martin to manage
his finances. This agency helps and benefits Martin as he is able to control his finances and
as his advisor got his permission, they contacted the landlord to discuss the rent arrears,
which makes stress decrease for Martin and this helped him a lot with his money problems.
Martin’s skin condition is worsening and has visited his G.P who then referred him to a
dermatologist at the NHS trust who has more facilities to help him with his skin condition by
providing the medicines he needs for it. The agencies involved can help improve his
condition and his mental state where he feels stressed by having the local authorities and
the clinical commissioning groups help Martin.


P7- Explain the roles and responsibilities of different members of the multidisciplinary team
in meeting the needs of specific individuals.

A multidisciplinary team is a trained group of people that work within the health and social
care sector buh have different roles so they can provide specific services for individuals with
different needs. They focus on each issue the individual may have and provide the right
specialist so they can come together as a team and create a personalised care plan for the
service user like they did with Nusrat.. An individual could possibly need more than one
discipline as they may need help with different issues. Multidisciplinary is effective as it
avoids the replication of roles and responsibility a professional may need to do. Examples of
a team of multidisciplinary members are: doctors, nutritionist, physiotherapist, nurses,
dietitian and so much more. Each one has different roles and responsibilities.

A General Practitioner (G.P) refers patients to the hospital if the patient is in a critical
condition and needs urgent treatment. The G.P is someone who you would approach first
and they would assess your condition and diagnose it and give u treatment such as
medication, however, sometimes they refer you to a hospital to get checked out by a
specialist. The G.P responsibility is to make sure the health of the patient is maintained.
They are meant to focus on the individual’s whole health and have an important role in
dealing with the problem by giving a solution. They deal with newborn babies to elderly and
look at overall health such as physical, mental and emotional.
