Individual Education Plan
This is a plan for children with SEN to help them get the most out of what they are learning.
This is a teaching and learning plan that consider where children are at in the learning and develop-

The purpose of this plan is to inform the teacher and others working with the child of specific targets
for the child.
Should contain: 3 or 4 short targets, provisions (actions to take) and outcomes
May also contain:
Likes/dislikes
Assessment information
Details of any other plan they might have
How the IEP will be co-ordinated
Child's additional support needs
Who will be providing the support
Home-based tasks and parent/child comments
Information and timetables for reviewing plan
Target to achieve in a certain time frame
