Patient experiencing mania or hypomania may employ the defense mechanism of denial It's difficult for manic / hypomanic individual to see or hear the symptoms as being abnormal. Person perceives manic / hypomanic symptoms as normal. The individual likes having higher energy, decreased need for sleep, and having an increase in activities and socialization Patient experiencing mania or hypomania may employ defense mechanism of reaction formation The individual feels depressed internally but displays a happy or energetic outward appearance Hyperactivity Restlessness Talkative Interrupting Difficulty with concentration Symptoms shared by bipolar & ADHD Labs / Diagnostic Testing: Rule out physical illness, such as hyperthyroidism Complete Blood Count (CBC) Labs / Diagnostic Testing: Rule out neurological disorders Electroencephalogram (EEG) Labs / Diagnostic Testing: Rule out underlying physical / neurological pathology in manic patients exhibiting aggressive behavior PET MRI CT scan X-ray Labs / Diagnostic Testing: Screen patient for possible STIs Rapid Plasma Reagin (RPR) - syphilis HIV test (various types of tests) Herpes (blood draw) Gonorrhea / Chlamydia (swab) Genital warts (visual inspection / biopsy & pathology) Labs / Diagnostic Testing: Screen patient alcohol or drug impairment Drug / toxicology panel It may be prudent to include STI screening for a patient exhibiting mania Manic mood increases risk taking behaviors, such as unsafe sexual encounters (Patients in manic phase can experience hypersexuality - greatly increased focus / desire for / interest in sexual gratification) Appropriate nursing intervention when patient is extremely delusional, labile, and unable to retain information Defer patient teaching until patient is able to engage in learning The nurse knows that manic patients can be easily distracted. To compensate, the nurse understands this is the appropriate intervention Plan to teach in a non-stimulating environment to lessen distractions & increase retention of information
