Preventive Care has three parts - Primary prevention (prevent disease or injury, such as
educating people not to smoke)
Secondary prevention (blocks progression of disease or injury such as taking a MRI)
Tertiary prevention (blocks or retard progression)
Hospitals and Nursing facilities use what type of prevention? - Secondary and Tertiary
Primary Care - Early Detection
Routine Care
Secondary Care (acute care) - Emergency Treatment
Critical Care
Tertiary/Quaternary Care (special care) - Highly technical services for example a cancer
hospital
Restorative Care - Rehabilitation
Home Care
Intermediate/Follow-up Care
Continuing Care - Long-term care
Chronic Care
Personal Care
Hospice/Palliative Care
Classification of HSOs - 1. Ownership
2. Length of Stay
3. Role in Health Service System
Ownership - 1. Profit
2. Not for Profit
3. Goverment Sponsored
For profit - Investor owned
Private Corporations - Can be for or not for profit
Goverment Sponsored - Classified as not for profit and owned by a public entity
Two types of privately owned corporations - Faith-based and nonsectarian that are
organized as not-for-profits
,For-profit corporations that issue stock to investors or to general public
Cities and counties - Can own acute care hospitals some of which are financed by
special tax districts
They establish, fund, and control public health departments
HSOs owned by the state government - Include health departments and psychiatric
hospitals
Federal Goverment has - limited involvement in financing health services
It has delivered preventive, acute, and long term health services to special groups. For
example the U.S Public Health Service (USPHS)
USPHS - U.S Public Health Service
Established in late 18th century to care for merchant sea men. They closed around the
1980s and were converted to other uses
Length of Patient Stay - Divid HSOs whether services are provided to:
1. Inpatients (treated 24 hours or longer)
2. Outpatients (ambulatory care)
Inpatient Care - Divided into:
1. Short term(acute) where average LOS is < 30 days
2. Long term- > 30 days
Average LOS in a Community Hospital (short term acute care) - 5.4 days
Long term care hospitals - provide extended medical and rehabilitative services to
patients . Usually chronic conditions
LOS in a Nursing Home - measured in months to years
Outpatients (ambulatory care) - Physicians offices
Hospital Emergency Department
Clinics
Surgery Centers
Imaging Centers
Home health services - A unique blend of inpatient and outpatient because care is
provided in patients home over months.
, Hospice is another example of this
Role in the Health Service System - classified by their role in delivery services
Mental Health Organizations - primarily provide mental health services to people with
mental illness or emotional disturbances
Teaching Hospitals - Many states own academic health (medical) centers, which are
often university-affiliated teaching hospitals that treat acute illness, conduct research,
and educate those in the health occupations
Hill-Burton Act of 1946 - included statewide planning for hospital services. Where
financial services were provided to build hospitals to improve healthcare
Professional Standards Review Organizations (PSROs) - Outcomes were measured
against performance standards
Officially renamed Quality Improvement Organizations (QIO) in 2001
Accreditation in Healthcare - 1. The Joint Commission
2. American Osteopathic Association(AOA)
3. Community Health Accreditation Program
4. International Organization for Standardization (ISO)
5. Educational Accreditors
6.Medical Specialization
The Joint Commission - Has expanded it services beyond hospitals it accredits nine
types of providers.
Accreditation of Networks ended (MCOs, preferred provider organizations) ended in
2006
Almost all hospitals are joint commission accredited
The Joint Commission has the following benefits: - 1. Provides a competitive edge in the
market place
2. Improves risk management and risk reduction
3. Strengthens patients safety efforts
HSO accredited by the Joint commission met the standards for: - 1. Patient safety
2. Provide education on good practice to improve business operations
3. Hold a competitive edge in the market place
American Osteopathic Association(AOA) - Osteopathic hospitals are accredited by the
AOA as well s the joint commission
