Assess for bleeding, how much is she bleeding? Correct
Answer After childbirth, it is critical that the uterine fundus
stay well contracted to clamp off uterine blood vessels at the
placen- tal site and thereby prevent hemorrhaging.

, Consequently, pal- pate the uterine fundus at frequent intervals
for the first 4 hours to ensure that it remains firmly contracted.
Normally, the fundus is located in the midline and at or below
the umbilicus. Palpate the fundus (Figure 18-10) but do not
massage it unless it is soft (boggy). When a uterus becomes
boggy, pooling of blood occurs within it, causing clots.
Anything left in the uterus prevents it from contracting
effectively. Thus, if the uterus becomes boggy or appears to rise
in the abdomen, massage the fundus until firm; then with one
hand supporting the uterus at the symphysis pubis, use the other
hand to exert steady pressure on the fundus to express retained
clots. The uterus is very tender at this time, so all palpation and
massage must be done as gently as possible.
In some women, the uterus becomes so relaxed that it can- not
be found when palpation is attempted. In this case, place one
hand in the midline of the abdomen at about the level of the
umbilicus and begin to make kneading motions. This motion
generally stimulates the uterus to contract, and it then feels like a
firm, hard object. (See Assessing the Uterine Fundus Following
Vaginal or Cesarean Birth in the Clinical Skills Manual SKILLS
.) . Mater- nal risks associated with induction of labor include
increased risk of postpartum hemorrhages, which increases the
risk for blood transfu- sions, hysterectomy, placenta
implantation abnormalities in future preg- nancies, a longer
hospital stay, more hospital readmissions, and,
in the worst case scenario, death (AWHONN, 2014)
