Healthcare Systems Applications C816

, The existing hybrid electronic health record (EHR) system at Anywhere Memorial Hospital

presents notable difficulties in clinical documentation. The system involves a combination of

digital and paper records, where some providers prefer manual documentation over computer-

based methods, relying on paper records and computer on wheels (COW) devices. This hybrid

approach leads to delays and inefficiencies as HIM employees must scan paper records into the

system. The reliance on paper documentation causes problems during system downtimes, with

healthcare providers facing difficulty accessing crucial patient information promptly. Moreover,

the manual retrieval of paper records by HIM staff for patient care units results in lost

documents and misplaced patient records, contributing to disorganization and hindering

effective patient care. The process of scanning and tracking paper records negatively impacts

turnaround times, revenue cycles, and the overall efficiency of the hospital's health information

management (Oachs & Watters, 2020).

An entirely digital record system will efficiently tackle the issues presented by Anywhere

Memorial Hospital's existing hybrid record system. Enabling healthcare providers to input

patient information directly into the system will eliminate the dependence on paper records

and the necessity for staff to scan and handle these documents. Real-time access to patient

information will empower physicians, particularly younger ones, to provide more immediate

and comprehensive care. The transition will significantly reduce the turnaround times for

completed medical records, improving the revenue cycle, and minimizing the risk of errors

associated with manual deficiency tracking. Automation of deficiency tracking in the digital

record will also ensure the timely completion of records and alert staff when necessary,

preventing delays and organizational inefficiencies. Furthermore, the digital record system will

enhance overall data
