Addiction Treatment Services Broad range of planned and continuing care, treatment,
rehabilitation, including but not limited to counseling, psychological, medical, and social service
care designed to influence behavior of individual alcohol abusers or drug abusers, based on an
individual treatment plan
Alcohol and Drug Services Program An entity to provide a service for persons charged
with or convicted of an infraction, misdemeanor, or felony and that provides substance abuse
assessment, intervention, education, referral treatment, or rehabilitation pursuant to IC-23-14
Assessment The process of evaluating and determining an appropriate level of
intervention based on information obtained from the client in a personal interview and from other
sources which may include substance abuse screening assessments
Case management Goal oriented activities that facilitate, coordinate, or monitor the full
range of basic human needs, treatment and service resources and delivery for individual alcohol
and drug program clients, which includes monitoring with notes in chronological order, with date
and initials or signature of staff

Client rights Client has right to:
a. Be informed of steps and activities involved in receiving services
b. confidentiality
c. make informed decision whether to participate in program or refuse participation
d. humane care and participation from harm, abuse and neglect
e. practice their religion
f. contact and consult with counsel and private practioners of client's choice
g. inspect and copy the client's record
Client assessment Includes:
a. statement of presenting problem
b. social/peer group
c. military history
d. suicide/homicidal thoughts and attempts
e. family history of substance use
f. occupation, education
g. mental health history
