This document explains the steps for the manned stations only/ briefly touch ups on unmanned, it
also goes through a frame which I created and used at my live on day assessment…
Achieved GRADE: PASS
There are six stations which are 10 minutes each:
Live: diagnosis, history taking, medicines counselling, brief intervention…
Unmanned which is done online: MI, medicines reconciliation…
Diagnosis station

Introduce yourself: Hello! My name is [your name] . I’m a student
pharmacist and can I confirm your name please? [asking all appropriate
questions] How would you like me to address yourself today? Right, that’s
great “say name” before you get into asking a few questions regarding
patient concerns today, do I have permission to conduct this consultation
today with yourself? Do you feel comfortable having a chat here or in a
private room? Okay that’s not a problem, [asking reason for consultation]
“How can I help you today?”

- Who’s the patient - can I just confirm the name of the patient I am
consulting today? For example, if someone is with patient, Are you mum
for “”
- What symptoms are you facing? Where is this located?
- How long has this been like this for?
- Have you done anything to resolve this?
- Are you on any medication at all? Any allergies?
1. Physical examination
2. Explaining condition - right ok so this can be treated in pharmacy, and
it is looking like they may have “” however this may not 100% be correct
due to a variety of symptoms shown. [signposting if needed]
General advice if required that can go along such as:
It is best to carry on with what you are doing such as paracetamol, Calpol
to reduce temp and pain if faced. And if you feel that his/her condition is
getting worse then please don’t hesitate to come back to me
- [closing the conversation] Thank- you so much for your time and
popping in if you have any other concerns or it’s getting worse and not
improving. Please do not hesitate to come back and see me or ultimately
refer to GP. [safety netting]
