WGU C792 Task 2: Entity Relationship Diagram. Throughout my career as a nurse, I've been aware of the importance of databases for tracking patient information. But, prior to this position, I had zero database experience. This project taught me a lot about databases, database design, and data modeling. DMBS is a piece of software designed to answer questions about data by storing it, retrieving it, and processing it. As an interface, the DMBS lets users add, view, modify, and remove records (AppDynamics, n.d.). Data modeling serves a purpose similar to that of DMBS in that it clarifies the connections between various data elements. What I've learned in this course is that data modeling makes it possible to store and retrieve data efficiently, and that it also reports the information that makes it possible to quickly retrieve data by selecting certain information. In a hospital database, for instance, we can view a patient's medical history with a single click, without having to open any additional files. Entity relationship diagrams like the one I made were possible because of my knowledge of how data are linked, which in turn makes for easy archiving and retrieval (ERD). For instance, a patient's name, Birthdate, and medical record number provide a three-part identifier in hospitals (MRN). In this scenario, patients, dates of birth, and medical record numbers are all aspects that interact to provide a unique identifier for each patient. Working here also taught me how data modeling can be used to help pinpoint where improvements are needed and where bottlenecks exist (Sap, n.d.). Why? Because a data model acts as a common language that various systems can speak to exchange and utilize the data as intended by the model. For instance, the nursing staff can take a model that has proven effective in one of the hospital's units to implement the same workflow design in another. With data modeling, you may have a better idea of whether or not this process needs any tweaks or improvements based on past performance. 4 Survey Interoperability Interoperability is an essential tool for facilitating the sharing of information between electronic health record systems. As defined by the Healthcare Information and Management Systems Society (HIMSS), interoperability is "the ability of different information systems, devices, and applications to access, exchange, integrate, and cooperatively use data in a coordinated manner, within and across organizational, regional, and national boundaries, to provide timely and seamless portability of information and optimize the health of individuals and populations globally" (HIMSS, n.d.). Interoperability aids in the provision of safe and effective patient care by enabling healthcare providers to access patient records and share information across multiple organizations. For example, a patient with acute appendicitis might be transferred from an urgent care center to the hospital (ED). As soon as they check into the hospital's emergency department (ED), doctors, nurses, and surgeons have instantaneous access to their records due to the interoperability of the two organizations' EHRs. This allows them to review previous diagnoses, notes, test findings, and diagnostic imaging, which eliminates the need to order duplicate labs and other procedures (Budhdeo et al., 2021). As a result, the patient can be taken to the operating room to undergo the necessary treatment without having to reschedule due to lack of information. In addition to improving the quality of care and patient outcomes, interoperability between healthcare providers’ EHRs also saves patients time and energy by not having to bring their medical records with them to each new clinic or hospital. Interoperability is a powerful tool for ensuring that the right information is shared across providers, which in turn helps to improve the quality of care for patients.
