Please do not use any of the Nurse Daniel information for your own topic, nursing intervention, or change project. Nurse Daniel serves as an example only to illustrate the change process. Name: Jessica Simmons Star Point 1: Discovery (Identify topic and practice issue) Identify the topic and the nursing practice issue related to this topic. (This MUST involve a nursing practice issue.) Topic is hospital acquired infections (HAIs) and the nursing practice issue is central lineassociated bloodstream infections (CLABSIs) and the high incidence in ICUs and Medical Stepdown Units Briefly describe your rationale for your topic selection. Include the scope of the issue/problem. The nursing practice issue I would like to investigate is hospital acquired infections (HAIs), specifically central line-associated bloodstream infections (CLABSIs). CLABSIs are critical to investigate because I believe that the number of instances in our hospital can not only be decreased but can be prevented almost entirely. Nurses have been educated on the possible causes as well as signs and symptoms of various HAI’s, including CLABSIs, and we can and need to be more proactive in our evaluations and treatment plans for those patients with central lines. Steps can be taken to ensure proper care of the central line that will in turn prevent the blood infections related to those lines from occurring. Establishing and enforcing dressing changes every seven days utilizing a dressing impregnated with CHG, needless valve changesevery 96 hours, daily hibiclens bathing for patients who have central lines, and “scrubbing the hub” with an alcohol wipe for at least fifteen seconds every time the hub is accessed are some of the interventions that can be used to prevent CLABSIs. CLABSIs contribute to extended hospital stays, increased costs for hospitals, and an increase in mortality among patients. According to the National Center for Biotechnology Information, around 250,000 bloodstream infections occur annually in the United States, with the majority being related to the presence of intravascular devices (Haddadin, 2020). Central lines come in two types, tunneled and non-tunneled, with non-tunnel catheters being the most commonly used and also accounting for the highest rates of infection. Infection occurs when bacteria on the skins surface migrate along the catheters external surface to the skin exit site toward the intravascular space. This migration route is why it is imperative to implement nursing interventions design to keep the catheter exit site and surrounding skin clean and free of harmful t Week 6 EBP Change Process FORM Jessica Simmons
