Nursing 6435 Week 8 Discussion What do you believe is your best course of action for this appointment? Children with seizure disorders are 15-19% more likely to drown despite taking seizure mediations as ordered (, 2019). Timmy is hypoxic and is at risk for respiratory failure. He requires supplemental oxygen and transfer to the hospital immediately via EMS. Respiratory failure occurs when one or both of the following are present, PCO260, -Type I, and or PAC02 is greater than 50, type II (emedicine-medscape,com, 2019). The WHO strongly recommends any child with SPO2 less than 94% be started on oxygen with continuous saturation monitoring (WHO, 2016). I would start Timmy on 5 liters of 02 via face mask with reservoir if available, due to his developmental disability and stay with him until EMS arrives. In prescribing 02 therapy, it is important to notice the signs of hyperoxemia and hypercapnia. Signs of hypercapnia include headache, behavior changes, papilledema, coma, asterixis and warm extremities (, 2019). Timmy should be admitted to the ICU, heart monitor applied, and continuous reassessments until stable. In the event that his oxygen saturations have not returned to 94%, CPAP should be started. If 02 saturations return then O2 should be monitored and titrated (, 2019).
