NURS 6541 WEEK 3 CASE STUDY Case Study #3 In the given case study, the father of a 10-year-old boy calls the office requesting assistance for an individualized educational plan (IEP). The child was recently diagnosed with attention deficit hyperactivity disorder. The father requests that the provider contact the school and facilitate an IEP being developed. For this discussion, confidentiality between patient and provider will be discussed along with possible interventions and management strategies for the patient. Confidentiality It is important for the provider to provide confidentiality regarding patient diagnosis and treatment as requested by the parents. This patient has been diagnosed with ADHD, and the father is requesting an IEP for the child at his school. The confidentiality regarding the patient and parent is not an issue because the parent is requesting this on behalf of the patient. The confidentiality issue comes to play with what to discuss with the school regarding the patient. Privacy rights allow parents to make decisions about their child’s care which can include immunization refusal and lifesaving treatments (Schapiro, 2009). Parents should be encouraged to review educational plans and advocate for the child (Burns, et al, 2017). Older children in some states can direct their medical care (Schapiro, 2009). Interventions and Management Strategies There are many interventions and strategies to implement when helping to obtain an IEP for a patient. Every child with a disability must be served by the local public-school district, and testing should be done without discrimination (Woodworth, 2016). An appointment should be made with the child and parent to discuss what the school has and hasn’t done for the child as of that time. The provider can notify the IEP team at the local school for the need of an evaluation of the student to determine need. The IEP team at the school usually consists of counselors, principal, and special education teachers (Woodworth, 2016). The provider should also discuss with the child any problems that the patient feels that they are having at school including grades and activities (Hagan, Shaw, & Duncan, 2017). It is a team effort to help the patient receive the best education possible. Conclusion IEP are different for any patient. The plan needs to be tailored to the child’s needs. Not all patients with a diagnosis of ADHD have poor grades, therefore weaknesses in the child’s education should be targeted. The provider and the parent should advocate for the child’s best interests. References: Burns, C. E., Dunn, A. M., Brady, M. A., Starr, N. B., Blosser, C. G., & Garzon, D. L. (Eds.). (2017). I work in emergency medicine. You would not believe the number of underage patients that come to the emergency room seeking treatment without a parent present. It is a sticky situation that is involved in that- do we contact parents for consent or do we treat without parental consent? Fortunately, a majority of the time the treatment needed is not necessarily emergent and we can call parents for consent. It is their insurance that the patient is using. However, if the patient is pregnant, there is no consent needed from the parent if it is regarding the patient’s pregnancy. Patients can request things to remain confidential, especially in situation like in this case (Burns et al, 2017). Mother or father that finds out about the situation would probably call later regarding this visit. What information can the provider give to this parent? Adolescent patients can request that test results remain anonymous in these situations. The provider can not inform the parents of these test results if requested (Schapiro, 2009). The problem then becomes dealing with angry parents. References: Burns, C. E., Dunn, A. M., Brady, M. A., Starr, N. B., Blosser, C. G., & Garzon, D. L. (Eds.). (2017).
