Week 10- SOAP Note Identifying data Name: Mary Jones Age: 41-year old Gender: Female Ethnicity: Caucasian S) CC: Finger pain HPI: Mary presents to the clinic with her son, Jack. She is complaining about her sore finger that will not heal. She has been using a topical triple antibiotic ointment on it and it seems to help for a while, but it always returns. She shares that she has never had this before. She reports no fever, but is concerned that it is now very tender, swollen and red. She took a picture of it when it was “not too bad 3 weeks ago [picture 1] … but it only seems to be getting worse!” Mary states her son, Jack is getting over a cold, but he is better now. “I take him to a play gym and he likely caught something there, but do you think I caught something from him?” She claims she has been changing and washing a lot of Jack’s clothes and linens on a daily basis with water, laundry detergent, fabric softener, and bleach as needed
