Medical Case 1: Kenneth Bronson Guided Reflection Questions 1. How did the scenario make you feel? This scenario made me feel a little nervous since the patient went downhill so quickly and he had originally said he had no allergies. In the real world this could happen at any time because people aren’t always aware of their allergies; especially if they are allergic to a medication they have never taken before. 2. What signs and symptoms led you to the conclusion that Kenneth Bronson was experiencing an allergic reaction? His heart rate increased quickly, he was having difficulty breathing, elevated blood pressure, upon auscultation it sounded like there was an obstruction. There was also a decrease in SPO2 even though he was on supplemental oxygen. 3. Discuss the differences between mild, moderate, and severe anaphylactic reactions. A mild reaction would include an itchy or runny nose, sneezing, itchy mouth, a few hives or mild itch and milk nausea or discomfort. Moderate reactions include cough or wheeze, diarrhea, sweating, pallor and tachycardia. Severe reactions include difficulty breathing, collapse, vomiting, shock, edema of the larynx and trouble speaking. 4. Discuss the importance of follow-up assessments post-reaction. It is important to conduct follow up assessments post reaction because the patient could have residual effects of anaphylaxis after the initial reaction that could still cause harm to the patient. 5. What further needs does Kenneth Bronson have at the end of the scenario that future nursing care should address? Kenneth Bronson needs to be educated on anaphylactic shock, signs and symptoms of a reaction and the importance of carrying an epi pen with him. 6. Reflect on how you would communicate with family members in an emergency situation if they were present at the bedside. I would be honest with the patient family and say he is having a reaction to the medication he just received, ask them to either leave the room or stand to the back so all members of the care team have room to work on the patient. 7. After completing the simulation and reflecting on your experience, what would you do differently (or the same) for the patient experiencing acute respiratory distress?
