He came in today for his two months follow - up. Mr. RT had a past medical history of HTN, CKD IV, vitamin D deficiency, and anemia. The patient was counseled extensively today regarding his non-compliancy with his medication regimen. HPI: Mr. RT is a 68-year-old African American male with a history of CKD stage 5, anemia of CKD and HTN. The patient has been followed closely by a nephrologist in Jacksonville. The patient was told six months ago that he was very close to start dialysis. He attended the Kidney Smart class, and he has decided to go on peritoneal dialysis. He has no uremic symptoms. His BP at home at home has been controlled. No constitutional symptoms. PMH:– HTN, Chronic Kidney Disease, Vitamin D deficiency, And Anemia in CKD. Allergies: No allergies to medications, food, or environmental allergies. Medications: Lisinopril 10 mg PO QD Vitamin D3 50, 000 IU PO QMonth Amlodipine 10 mg PO QD Veltassa 8.4 g PO QD Viagra 100 mg PO QD Sodium bicarbonate 650 mg PO TID
