Health History
Demographics
YB is a 35-year-old Hispanic, female who resides in the Manhattan neighborhood outside
the city of Chicago, Illinois. YB’s date of birth is January 02, 1988. YB was born in Zacatecas,
Mexico and raised in the suburbs or Chicago, Illinois. YB was raised in a home with both parents
and two younger sisters. YB has some current medical complaints.
Perception of Health
When asked how YB views her health, she states “I try to be as healthy as I can, I eat
organic and clean, and attend Pilates class at least three times a week”. YB was diagnosed with
hypothyroidism seven years ago and recently found out she is allergic to several foods and
environmental factors. After YB’s diagnosis, she changed her eating habits and drinks alcohol
socially. YB’s avoids eating fried food, sunflower oil, sweets, and unhealthy chips.
Past Medical History
YB had chicken pox at the age of 4 years old, she had no other illnesses growing up. She
received all of her vaccines either here in the United States or in Mexico. YB has had no
surgeries. YB has had one hospital stay due to a severe migraine and two days after giving birth
to her daughter. YB has hypothyroidism and is currently not taking medication for it and taking
a holistic approach.
