🦠 PILLAR 1: CORE COMPREHENSIVE NOTES – SYSTEMIC
MICROBIOLOGY 🦠
⚠️ Core Instructions for Students: This workspace serves as your master
high-yield matrix for USMLE Step 1 Microbiology. Do not waste time trying to
memorize phone-book style detail. Focus entirely on the architectural
structure, distinct biochemical identification profiles, and the specific
molecular mechanics of virulence engines and toxin cascades that the boards
love to test. Everything below integrates seamlessly with First Aid and UWorld
paradigms.
🧫 MODULE A: GRAM-POSITIVE COCCI HUB

, 1. Staphylococcus aureus (The Catalase & Coagulase Positive
Titan)
Identification Profile: Gram-positive cocci arranged in clusters. Catalase-
positive (breaks down H₂O₂ into water and oxygen; invokes the PLACESS
rule) and Coagulase-positive (directly activates clotting by converting
fibrinogen to fibrin, creating a protective coating matrix against phagocytosis).
Shows brilliant β-hemolysis (clear zones of complete RBC destruction) on
blood agar and ferments mannitol (turning the plate agar yellow) on Mannitol
Salt Agar (MSA).
Virulence Engine:
Protein A: The ultimate stealth mechanism. Protein A binds directly to the
Fc region of IgG at the exact site where complement normally attaches.
Molecular Mechanics: This flips the antibody backwards. The macrophage
cannot recognize the flipped antibody, completely blocking opsonization,
complement activation, and host phagocytosis.
Toxin Vault & Board Correlations:
TSST-1 (Toxic Shock Syndrome Toxin-1): A powerful Superantigen. It
forces a non-specific cross-linking of the MHC Class II receptor on
antigen-presenting cells to the T-cell receptor (TCR) outside the standard
antigen-binding groove.
Molecular Mechanics: This massive bridge bypasses normal immune
check-gates, causing an explosive activation of up to 20% of all naive T-
cells. This triggers a massive, fatal systemic cytokine storm driven by IFN-
gamma and IL-2, presenting as high fever, diffuse sunburn-like rash
(including palms/soles), and hypotensive shock.
Exfoliative Toxin: A highly specific protease that targets cell adhesion. It
cleaves desmoglein-1 in the stratum granulosum layer of the epidermis.
This drives Staphylococcal Scalded Skin Syndrome (SSSS) in infants,
marked by diffuse skin sloughing and a positive Nikolsky sign.
