MSN 570 WEEK 6 CASE STUDY GUIDE AND
COMPLETE SOLUTION
A 19 year old presents with a deep laceration on the left leg. After
sutures the patient is discharged.

1. Discuss the pathophysiology of wound healing.

There are four basic phases of wound healing: 1. Hemostasis, 2.
Inflammatory, 3. Proliferative, 4. Remodeling or maturation. During
Hemostasis, vasoconstriction occurs to prevent the body from bleeding
out rapidly, this is achieved by platelets delivered to the site to form a
clot. Next, the inflammatory cells are activated, and the wound starts to
close. Neutrophils, cytokines and mediators promote the development
of new blood vessels, thrombosis, and epithelialization (desJardins-
Park,et al., 2018). Fibroblasts occur during this stage and are critical in
supporting normal wound healing. Fibroblasts break down the fibrin
clot, creating a new extracellular matrix, and collagen structures to
support the other cells (desJardins-Park et al.,2018). The inflammatory
stage can last many days, and is where many chronic wounds cannot
get past. The proliferative stage is where neovascularization and re-
epithelialization occur, and granulation tissue forms (Singh &
McNaught,2017). Cells involved in this phase include- macrophages,
fibroblasts, endothelial cells, mast cells, b-cells and myofibroblasts;
these different cells help recover the structure and function and
foundation of the wounded skin (Singh & McNaught, 2017). Excess
collagen will begin to degrade, and wound contraction begins to peak,
that’s where the final stage takes place. The remodeling phase usually
takes place during the 3rd week, and can last up to 12-24 months,
depending on the patient’s age, comorbidities, etc. The remodeling of
the wound continues to contract, fibers reorganize, and ultimately a
scar forms (desJardins-Park et al.,2018).
