Questions with Verified Correct Answers

Define clinical judgment. Compare and contrast clinical judgment and clinical reasoning.

Clinical judgment is an interpretation or conclusion about a patient's needs, concerns, or health
problems, and/or the decision to take action (or not), use or modify standard approaches, or
improvise new ones as deemed appropriate by the patient's response.
Clinical reasoning is the thinking process by which a nurse reaches clinical judgment; iterative
process of noticing, interpreting, and responding, depending on the patient and how they
respond.

Discuss the 3 defining attributes of the interpretivist perspective of clinical judgment.

1. Holistic view of the patient situation: all factors involved in the patient.
2. Process orientation: deep understanding of the patient as an individual (background,
experience, values).
3. Reasoning and interpretation: (3 types of reasoning: analytic, intuitive, narrative)

Define noticing.

Skill of recognizing a pattern of cluster of cues that signals one concern. Factors that influence
what a nurse notices includes Characteristics of the Nurse, Theoretical and Experiential
Knowledge of the Nurse, Knowing the patient, and context or environment of care.

Define interpreting.

The skill of assembling all the information to make sense of it. Students rely on analytical
reasoning. Expert nurses uses experiences, known as intuitive reasoning. A narrative reasoning
is making sense of a situation through the use of stories.

Define responding.

After collecting and interpreting the information, the nurse's action in a particular patient's
issue through one or more interventions is called a response. Experience helps the nurse move
from a more analytic way of thinking to a more intuitive/narrative.

Define reflecting (reflection-in-action and reflection-on-action).

, Ability to learn from one's actions
Reflection-in-action: refers to nurse's understanding o patient responses to nursing action while
care is occurring.
Reflection-on-action: consideration of the situation after the patient care occurs.

Explain reflection and how it is used in nursing, especially with clinical judgment.

Reflecting helps students to process and consolidate information. It is important to reflect on
successful and unsuccessful interventions and look for a need of improvement and how that will
happen.

Identify the ways that nurses make judgments.

What's the difference between responsibility and accountability?

Responsibility is the knowledge that you are accountable for your decisions, actions, and critical
thinking. Accountability involves recognizing when nursing care is ineffective and you know the
limits and scope of your practice.

Describe the 3 ways in which a nurse develops critical thinking skills.
