1. Fundamentals of Nursing

The foundation of nursing practice relies on accurate assessment, understanding baseline norms,
and implementing the nursing process effectively.

Normal Vital Signs by Age

Age Group Blood Pressure (mmHg)

Neonate (0-28 Systolic: 60 - 90 / Diastolic: 20

🚨 NCLEX ALERT: VITAL SIGNS EXCEPTIONS

Athletes may have a baseline bradycardia (HR < 60). Always assess the patient for symptoms
(dizziness, fatigue, chest pain) before treating abnormal numbers. Treat the patient, not the
monitor!

The Nursing Process (ADPIE)

The systematic framework for delivering nursing care.

• A - Assessment: Collecting subjective (what the patient says) and objective (what you
observe/measure) data. Always assess before taking action unless it's a known emergency!

• D - Diagnosis: Analyzing the data to identify actual or potential health problems.

• P - Planning: Setting SMART goals (Specific, Measurable, Attainable, Realistic, Timely).

• I - Implementation: Executing the nursing interventions.

• E - Evaluation: Determining if goals were met and modifying the plan if necessary.

High-Yield Patient Positioning

Position Description Common Indications

Supine Flat on back Spinal cord injury (no pillows), physical examination.

ARDS (improves oxygenation), post-lumbar puncture
Prone Flat on abdomen (prevents CSF leak), post-amputation (prevents hip
flexion contractures).

Semi-Fowler's

, Position Description Common Indications

Head of bed Promotes lung expansion, enteral feedings (prevents
elevated 30-45° aspiration).

Head of bed
High Fowler's Severe dyspnea, inserting NG tube, eating.
elevated 60-90°

Head lower than Hypotension/Shock (promotes venous return), inserting
Trendelenburg
feet central line.

Lateral / Side- Lying on left or Seizures (prevents aspiration), pregnancy (left side
Lying right side relieves vena cava compression).

2. High-Yield Pharmacology

Pharmacology is heavily tested. Mastering prefixes, suffixes, and antidotes is the most efficient
way to study.

Must-Know Drug Endings

Suffix/Prefix Drug Class Key Nursing Considerations

ACE Inhibitors Watch for dry cough, hyperkalemia, angioedema.
-pril
(Antihypertensive) (e.g., Lisinopril)

Alternative if patient develops ACE inhibitor cough.
-sartan ARBs (Antihypertensive)
(e.g., Losartan)

Beta Blockers Check HR and BP before giving. Can mask signs of
-lol
(Antihypertensive) hypoglycemia. Avoid in asthma. (e.g., Metoprolol)

Calcium Channel Monitor for peripheral edema, constipation. Avoid
-dipine
Blockers grapefruit juice. (e.g., Amlodipine)

HMG-CoA Reductase Monitor liver enzymes. Watch for muscle pain
-statin
Inhibitors (rhabdomyolysis). Give at bedtime. (e.g., Atorvastatin)

, Suffix/Prefix Drug Class Key Nursing Considerations

Proton Pump Inhibitors Give 30 mins before meals. Long-term use increases
-prazole
(PPIs) risk of osteoporosis and C. diff. (e.g., Omeprazole)

Check for allergies (cross-sensitivity with
-cillin Penicillin Antibiotics
cephalosporins). (e.g., Amoxicillin)

Ototoxicity (hearing loss/tinnitus) and
Aminoglycosides /
-mycin Nephrotoxicity. Check peak/trough levels. (e.g.,
Macrolides
Gentamicin)

-pam / - CNS depressant. High risk for addiction/withdrawal.
Benzodiazepines
lam Monitor respiratory rate. (e.g., Diazepam, Alprazolam)

💡 HIGH-YIELD ANTIDOTES

Memorize these specific reversal agents:

• Opioids (Morphine, Fentanyl): Naloxone (Narcan)

• Benzodiazepines: Flumazenil (Romazicon)

• Warfarin (Coumadin): Vitamin K

• Heparin: Protamine Sulfate

• Acetaminophen (Tylenol): Acetylcysteine (Mucomyst)

• Digoxin: Digibind / Digoxin Immune Fab

• Magnesium Sulfate: Calcium Gluconate

🚨 DIGOXIN TOXICITY WARNING

Therapeutic range is narrow (0.5 - 2.0 ng/mL). Early signs of toxicity include GI upset
(anorexia, N/V). Late signs include visual changes (yellow/green halos). Hypokalemia
increases the risk of digoxin toxicity. Always check apical pulse for 1 full minute before
administration (hold if < 60 bpm in adults).
