, A1. A hybrid record is a form of storing patient information using a combination of
paper and Electronic Health Record (EHR). While using this system the Health Information
Management (HIM) are primarily responsible for gathering, tracking, and storing patient
information in many places and formats (Medesk., n.d.). A few issues with this system are
how slow turnaround times for health records, inconsistent in documentation, and is
subjected to human error of misplacing a document.
Hypothetically, if a patient has been seen by multiple doctors in the Emergency
Room (ER) that use a hybrid record you could lose continuation of care since some doctors
could be charting in a paper record, others my only be looking in the EHR. Hybrid records
are hard for healthcare providers to have access to up to data information since Health
Information Management (HIM) employees manually gather and scan each paper before it
is out into the health record. Hybrid records result in delinquent care since the provider
doesn’t always have the information needed to perform a complete assessment. During
times when the documentation scanner is down, the providers will experience delays. The
HIM staff would have to supply the nursing floors with paper documentation risking lost or
misplaced information.
Depending on if this becomes a legal matter, the HIM department would have
a short amount of time to gather all paper records and scan them into the EHR system,
which results in slow turnover times for the complete medical record, which could lead to
inconsistencies within the health record.
A2. A full electronic health record is where the patient’s data is 100% stored on the
computer. This will allow for structured documentation which will include drop down boxes
and close ended questions. All laboratory reports, medications, doctor’s orders, and
appointments will be in one place for consistent patient information. Fully digital records
are easily accessible by clinicians, nursing staff, coders, and billing specialists. Since the
patient’s information will be submitted directly into the Electronic Health Record (EHR) it
increases revenue cycles and helps to keep the records organized, which in turn increase
efficiency. Hybrid records cause many problems for all employees since the HIM employee
