Client is a 22-year-old female reporting for counseling following a DUI arrest five

months ago. Client was driving a blood alcohol level of 0.13. Client was also charged possession

of 1 gram of marijuana. Currently client smokes one pack of cigarettes a day, smokes marijuana

two to three times per week, and drinks four or more alcoholic beverages three to four times per

week. Her usual pattern involves binging on alcohol and marijuana throughout each weekend,

with a typical Saturday evening consisting of six to seven alcoholic beverages and several shared

marijuana joints. During the past two months her drinking has increased to as many as four

glasses of wine every night, while alone in her room. Additionally, she has distanced herself

from her parents over her drinking and drug usage and has begun lying to them to cover up her

actions. Client has recently begun expressing concerns of “feeling depressed and anxious” but

reports no suicidal ideations. Client is also concerned that she has missed her period. Client has

never received treatment for drugs or alcohol and is in good health. The primary issue of concern

is the client’s increasing drug and alcohol use. She has already suffered a number of serious

consequences, to include 3 separate vehicular incidents, a loss of relationship with her family,

and a drop in academic achievement. Client’s anxiety and depression are likely being caused by

her drug use, despite her claims that it relaxes her.

Past and Current Substance Used and Their Effects

Client has a fairly extensive history of drinking and drug use begging when she was 13. Drugs

listed include:

Alcohol: consumed orally, leading to a feeling of detachment and apathy, allowing client to

disengage from her feelings of depression and anxiety. Negative effects can include nausea,

vomiting, loss of consciousness, lethal overdose, headaches and withdrawal symptoms, and give

the client’s history, the likelihood that she may attempt to operate a motor vehicle.
