Institutionalisation

Institutionalisation is the process through which individuals become dependent
on institutional environments and routines after spending prolonged periods in
settings such as hospitals, residential care homes, prisons or other long-term
care facilities. Over time, individuals may lose confidence, independence and
opportunities to make decisions for themselves as daily activities become
structured and controlled by institutional routines. Institutionalisation can affect
psychological wellbeing, social functioning and the ability to live independently
within the community. Within social work, understanding institutionalisation is
essential because practitioners seek to promote autonomy, community inclusion
and person-centred support while avoiding unnecessary dependence on
institutional care (Goffman, 1961).

The concept of institutionalisation was developed through sociological and
psychological research during the twentieth century. One of the most influential
contributors was Erving Goffman, who introduced the concept of the total
institution. Goffman described institutions such as psychiatric hospitals, prisons
and residential facilities as environments where all aspects of daily life occur in
the same place under a single authority, with highly structured routines and
limited personal freedom. He argued that these environments often reduce
individuality by imposing uniform rules, limiting personal choice and separating
individuals from wider society. As a result, residents may gradually adapt to
institutional life and become less able or less confident to function independently
outside the institution (Goffman, 1961).

Research into institutionalisation was further influenced by studies examining
the effects of long-term residential care on children and adults. John Bowlby's
work on attachment highlighted the developmental impact of institutional care
on children who experienced limited emotional relationships during early
childhood, demonstrating that prolonged institutional care could affect emotional
development, attachment and later relationships (Bowlby, 1969). Subsequent
research contributed to major reforms in child welfare and highlighted the
importance of family-based care and stable relationships.

Institutionalisation is characterised by several common features. Individuals may
become increasingly reliant on staff for everyday tasks, lose opportunities to
make independent decisions and experience reduced confidence in managing
daily life. Personal identity may also be affected as institutional routines replace
