Abstract
This academic review examines the nursing care of Ola, a 15-year-old transgender
adolescent admitted to the Emergency Department following a suicide attempt
precipitated by familial rejection of gender transition plans. The analysis integrates three
critical dimensions of mental health nursing: prioritized safety interventions for suicidal
patients, gender-affirming care as an evidence-based clinical strategy, and the influence
of Social Determinants of Health (SDOH) and implicit bias on adolescent psychiatric
outcomes. The Oxford Medical Simulation highlights the immediate need for 1:1 suicide
precautions, environmental safety sweeps, and comprehensive psychosocial risk
assessment. Gender-affirming nursing practices—including consistent use of preferred
names and pronouns—are examined as interventions that measurably reduce depressive
symptoms and suicidal ideation among transgender youth. The paper further addresses
how SDOH, particularly family acceptance and housing safety, determine discharge
readiness and the necessity of multidisciplinary collaboration with social workers,
psychiatrists, and LGBTQ+ support organizations.

1. Introduction
Lesbian, gay, bisexual, transgender, and queer/questioning (LGBTQ+) adolescents experience
disproportionately high rates of mental health disorders, with transgender youth reporting suicide
attempt rates estimated at 30 to 50 percent—a figure substantially exceeding that of cisgender peers
[1]. The NUR 302 Oxford Medical Simulation (OMS) featuring Ola, a 15-year-old assigned male at
birth (AMAB) with documented gender dysphoria and depression, provides a high-fidelity clinical
environment for nursing students to practice the integration of psychiatric safety protocols,
therapeutic communication, and gender-affirming care. This paper reviews the key clinical, ethical,
and social dimensions of the simulation, organized around three pillars: prioritized nursing
interventions for suicide risk management, evidence-based gender-affirming nursing practices, and
the influence of SDOH and implicit bias on clinical decision-making and discharge planning.
2. Prioritized Safety and Psychiatric Interventions
The immediate clinical priority for any patient presenting after a suicide attempt is safety. For
Ola, this begins with the initiation of strict, continuous 1:1 visual monitoring, ensuring the
adolescent is never left unattended during the initial assessment and stabilization period [2].
Concurrently, a thorough environmental sweep of the patient’s belongings and the hospital room
must be conducted to remove all potential hazards, including sharps, cords, belts, drawstrings, and
unnecessary medications. Providing a hospital safety gown further eliminates ligature risk
associated with the patient’s own clothing.
Comprehensive mental health assessment follows the establishment of physical safety. A direct,
non-judgmental suicide risk assessment should address three critical questions: whether the patient
is currently experiencing thoughts of self-harm, whether a specific plan exists, and whether the
